牙周基础治疗对慢性牙周炎合并2型糖尿病患者牙周炎症、血糖代谢水平及血清炎性指标的影响

Influence of periodontal basic therapy on periodontal inflammation, blood glucose metabolism level and serum inflammatory indexes in patients with chronic periodontitis and type 2 diabetes mellitus

目的考察牙周基础治疗对慢性牙周炎合并2型糖尿病患者牙周炎症、血糖代谢水平及血清炎性指标的影响。方法选取2017年6月~2018年5月在广东省江门市中心医院进行治疗的116例慢性牙周炎合并2型糖尿病患者,采用随机数字表法分为治疗组与观察组,各58例。治疗组进行牙周基础治疗,观察组进行常规治疗。比较两组患者治疗前后的牙周袋深度(PD)、附着丧失(AL)、空腹血糖(FBG)、糖化血红蛋白(HbA1c)、肿瘤坏死因子-α(TNF-α)和超敏C反应蛋白(hs-CRP)差异。结果两组患者治疗后的PD、AL小于治疗前,差异有统计学意义(P<0.05);两组患者治疗后的FBG、HbA1c水平低于治疗前,差异有统计学意义(P<0.05);两组患者治疗后的TNF-α、hs-CRP水平低于治疗前,差异有统计学意义(P<0.05);治疗组治疗后的PD、AL小于观察组,差异有统计学意义(P<0.05);治疗组治疗后的FBG、HbA1c水平低于观察组,差异有统计学意义(P<0.05);治疗组治疗后的TNF-α、hs-CRP水平低于观察组,差异有统计学意义(P<0.05)。结论与常规治疗相比,慢性牙周炎合并2型糖尿病患者采用牙周基础治疗,能明显改善牙周炎症状况、糖代谢水平和血清炎性指标水平,值得临床推广。

Objective To investigate the influence of periodontal basic therapy on periodontal inflammation,blood glucose metabolism and serum inflammatory indexes in patients with chronic periodontitis complicated with type 2 diabetes mellitus.Methods From June 2017 to May 2018,116 patients with chronic periodontitis complicated with type 2 diabetes treated in Central Hospital of Jiangmen City in Guangdong Province were chosen into the research.The patients were divided into the treatment group(n=58)and the observation group(n=58)according to random number table method.The treatment group received periodontal basic therapy,while the observation group received routine treatment.The differences of periodontal pocket depth(PD),attachment loss(AL),fasting blood glucose(FBG),glycosylated hemoglobin(HbA1c),tumor necrosis factor-α(TNF-α)and high sensitive C-reactive protein(hs-CRP)between the two groups before and after treatment were compared.Results The PD,AL after treatment in the two groups were significantly lower than those before treatment,the differences were statistically significant(P<0.05).The level of FBG,HbA1c after treatment in the two groups were significantly lower than those before treatment,the differences were statistically significant(P<0.05).The levels of TNF-αand hs-CRP after treatment in the two groups were significantly lower than those before treatment,the differences were statistically significant(P<0.05).The PD,AL in the treatment group after treatment were lower than those in the observation group,the differences were statistically significant(P<0.05).The level of FBG,HbA1c after treatment in the treatment group were lower than those in the observation group,the differences were statistically significant(P<0.05).The levels of TNF-αand hs-CRP after treatment in the observation group were significantly lower than those in the observation group,the differences were statistically significant(P<0.05).Conclusion Compared with routine treatment,periodontal basic therapy in patients with chronic periodontitis combined with type 2 diabetes mellitus can obviously improve periodontal inflammation,glucose metabolism level and serum inflammatory index level,which is worth popularizing in clinical practice.
