单侧腰麻在股骨头置换术老年患者中的应用效果

Application effect of unilateral lumbar anesthesia in elderly patients undergoing femoral head replacement

目的分析对接受股骨头置换术治疗的老年患者实施单侧腰麻的临床效果及对心血管系统产生的影响。方法选取2016年11月~2018年11月在我院接受股骨头置换术的60例老年患者,通过随机分组的方式分成对照组(30例)和研究组(30例)。对照组在手术期间实施硬膜外麻醉;研究组在手术期间实施单侧腰麻。比较两组手术麻醉效果、麻醉原因导致的不良反应情况、麻醉前后心功能相关指标变化幅度、麻醉苏醒时间和住院时间。结果研究组手术麻醉总有效率高于对照组;研究组麻醉原因导致的不良反应发生率为3.3%,低于对照组的20.0%;麻醉后心功能相关指标水平低于对照组;麻醉苏醒时间和住院时间短于对照组,差异均有统计学意义(P<0.05)。结论对接受股骨头置换术治疗的老年患者实施单侧腰麻,能够提高手术麻醉效果,减少麻醉原因导致的不良反应,对心脏功能产生的影响程度相对较小,可以在最大程度上缩短麻醉苏醒时间和住院治疗时间。

Objective To analyze the clinical effect of unilateral lumbar anesthesia on elderly patients undergoing femoral head replacement and its influence on cardiovascular system.Methods A total of 60 elderly patients who underwent femoral head replacement in our hospital from November 2016 to November 2018 were selected and randomly divided into control group(30 cases)and research group(30 cases).The control group received epidural anesthesia during operation,while the study group received unilateral lumbar anesthesia during operation.The effects of anesthesia,adverse reactions caused by anesthesia,changes of cardiac function related indexes before and after anesthesia,recovery time of anesthesia and hospitalization time were compared between the two groups.Results The total effective rate of operation anesthesia in the study group was higher than that in the control group;the incidence of adverse reactions caused by anesthesia in the study group was 3.3%,lower than 20.0%in the control group;the level of cardiac function related indexes after anesthesia was lower than that in the control group;the recovery time and hospitalization time of anesthesia were shorter than those in the control group,the differences were statistically significant(P<0.05).Conclusion Unilateral lumbar anesthesia for elderly patients undergoing femoral head replacement can improve the anesthetic effect of operation,reduce adverse reactions caused by anesthesia,and have a relatively small impact on cardiac function.It can shorten the recovery time of anesthesia and hospitalization time to the greatest extent.
