中药序贯治疗联合DA化疗方案治疗成人急性髓细胞白血病减毒增效效果观察

Effect of synergism and attenuation of sequential treatment of Chinese medicine combined with DA chemotherapy on adult acute myeloid leukemia

目的 观察中药序贯治疗联合DA化疗方案治疗成人急性髓细胞白血病(AML)减毒增效效果。方法 将60例成人AML患者按照随机数字表法分为2组。对照组30例行DA化疗方案;治疗组30例在对照组治疗基础上加中药序贯治疗。2组均30d为1个周期,2个周期后比较2组近期疗效、不良反应及毒副作用。结果 治疗组食欲减退、呕吐及肝功能异常发生率低于对照组(P<0.05),反应程度较对照组轻(P<0.05)。化疗过程中治疗组出血、感染及骨髓抑制发生率低于对照组(P<0.05)。化疗后2组白细胞计数(WBC)、血红蛋白(Hb)及血小板计数(PLT)均较本组化疗前降低(P<0.05),化疗后治疗组WBC、Hb及PLT水平高于对照组(P<0.05)。治疗组骨髓抑制时间较对照组短(P<0.05),WBC减少持续时间及粒细胞减少持续时间短,机采血小板量及升白药、悬浮红细胞、红细胞输入量少,比较差异均有统计学意义(P<0.05)。结论 中药序贯治疗联合DA化疗方案治疗成人AML能提高近期疗效,减轻化疗的毒副作用。

Objective To observe the effect of synergism and attenuation of sequential treatment of Chinese medicine combined with DA chemotherapy on adult acute myeloid leukemia (AML). Methods 60 AML patients were divided into two groups according to random number table method. 30 cases in control group were treated by DA chemotherapy. 30 cases in treatment group were treated by sequential treatment of Chinese medicine on the basis of the control group. The short-term efficacy, adverse reactions and side effects before and after chemotherapy were compared between the two groups. Results The incidence of anorexia, vomiting and abnormal liver function in the treatment group was lower than that in the control group ( P <0.05). The incidence of hemorrhage, infection and bone marrow suppression in the treatment group was significantly lower than that in the control group ( P <0.05). After chemotherapy, WBC, Hb and PLT in the two groups were lower than those in the group before chemotherapy ( P <0.05), and the treatment group were higher than the control group ( P <0.05). The time of bone marrow suppression in the treatment group was shorter than that in the control group ( P <0.05), the duration of WBC reduction and granulocytopenia was shorter, and the amount of platelets collected by machine, leucocytes, suspended red blood cells and red blood cell input were less than those in the control group ( P <0.05). Conclusion Sequential treatment with Chinese medicine combined with DA chemotherapy in the treatment of adult AML can improve the short-term efficacy and reduce the toxicity and side effects of chemotherapy.
