健脾益肾利湿通络中药汤剂联合针灸治疗痛风性关节炎疗效观察

Efficacy of Jianpi Yishen Lishui Tongluo Decoction Combined with Acupuncture and Moxibustion on Gouty Arthritis

目的:观察健脾益肾利湿通络中药汤剂联合针灸治疗痛风性关节炎(GA)的疗效。方法:收集2015年4月~2018年4月来我科就诊的GA患者90例,按照随机数字表随机分为观察组与对照组各45例,对照组给予秋水仙碱联合别嘌醇治疗,观察组给予健脾益肾利湿通络汤联合针灸治疗,连续治疗4周后,比较两组中医症候积分、实验室指标[红细胞沉降率(ESR)、血尿酸(UA)、C反应蛋白(CRP)]、疗效及不良反应。结果:治疗后,两组中医症候积分均显著低于治疗前(P<0.05),且观察组改善程度大于对照组(P<0.05);两组ESR、UA、CRP水平均显著低于治疗前(P<0.05),且观察组降低幅度大于对照组(P<0.05);观察组总有效率97.78%,显著高于对照组的80.00%(P<0.05);观察组不良反应发生率2.22%显著低于对照组的22.22%(P<0.05)。结论:健脾益肾利湿通络汤联合针灸用于GA患者治疗中,可以更好减轻患者中医症候,改善患者血尿酸水平及炎性反应指标,提高临床疗效,且不良反应少。

Objective:To observe the efficacy of Jianpi Yishen Lishui Tongluo Decoction combined with acupuncture and moxibustion on gouty arthritis(GA).Methods:90 cases patients with GA admitted to my section from April 2015 to April 2018 were collected and divided into observation group and controlled group according to the random number table method.The controlled group was treated with Colchicine combined with Allopurinol,and the observation group was given Jianpi Yishen Lishui Tongluo Decoction combined with acupuncture and moxibustion.After 4 weeks of continuous treatment,the TCM syndromes scores and laboratory indexes[erythrocyte sedimentation rate(ESC),blood uric acid(UA),C-reactive protein(CRP)],efficacy and adverse reactions were compared between the two groups.Results:After treatment,the TCM syndromes scores in the two groups were significantly lower than those before treatment(P<0.05),and the improvement in the observation group was greater than that in the controlled group(P<0.05).The levels of ESR,UA and CRP in the two groups were significantly lower than those before treatment(P<0.05),and the decrease in the observation group was greater than that in the controlled group(P<0.05).The total effective rate in the observation group was significantly higher than that in the controlled group(97.78%vs 80.00%)(P<0.05).The incidence rate of adverse reactions in the observation group was lower than that in the controlled group(2.22%vs 22.22%)(P<0.05).Conclusions:Jianpi Yishen Lishi Tongluo Decoction combined with acupuncture and moxibustion for patients with GA can better alleviate TCM symptoms,improve blood uric acid level and inflammatory response indicators,and promote clinical efficacy,and it has few adverse reactions.
