集束化护理策略干预应用于腹腔镜胃癌围术期中对术后康复、生活质量及并发症的影响

Effect of intervention of clustering nursing strategies on postoperative rehabilitation, quality of life and complications in laparoscopic gastric cancer during perioperative period

目的探讨集束化护理策略干预应用于腹腔镜胃癌围术期中对术后康复、生活质量及并发症的影响。方法选取该院2015年9月~2016年9月收治的行常规护理干预的腹腔镜胃癌手术患者61例作为对照组,另选取该院2016年10月~2017年10月收治的行集束化护理策略干预的腹腔镜胃癌手术患者61例作为观察组。比较两组术后康复情况、并发症发生率及护理前后生活质量变化。结果观察组肠鸣音恢复时间、首次排气时间、胃管留置时间及住院时间均短于对照组,差异有统计学意义(均P<0.05);观察组口干口臭、腹胀、恶心呕吐、便秘发生率均低于对照组,差异有统计学意义(均P<0.05);观察组护理1周后生理功能、生理职能、情感职能、活力、躯体疼痛、总体健康、精神健康、社会功能评分均高于对照组,差异有统计学意义(均P<0.05)。结论集束化护理策略干预应用于腹腔镜胃癌手术患者围术期中可有效促进其术后康复,降低并发症发生率,提高生活质量,值得临床推广。

Objective To explore the influence of clustering nursing strategy intervention on postoperative recovery, quality of life and complications in laparoscopic gastric cancer during perioperative period. Methods A total of 61 patients undergoing laparoscopic gastrectomy for gastric cancer from September 2015 to September 2016 in this hospital were selected as the control group.Another 61 cases of laparoscopic gastric cancer surgery patients admitted by this hospital from October 2016 to October 2017 who received cluster nursing strategy intervention were selected as the observation group.The postoperative recovery, incidence of complications, and quality of life before and after nursing were compared between the two groups. Results The recovery time of bowel sounds, time of first discharge, retention time of gastric tube and length of stay in observation group were shorter than those in control group , and the differences were statistically significant(P<0.05). The incidence of dry halitosis, abdominal distension, nausea, vomiting and constipation in the observation group was lower than that in the control group, and the difference was statistically significant(P<0.05). The scores of physiological function, physiological function, emotional function, vitality, physical pain, general health, mental health and social function in the observation group were all higher than those in the control group after 1 week of nursing, and the differences were statistically significant(P<0.05). Conclusions Intervention of clustering nursing strategy in the perioperative period of laparoscopic gastric cancer patients can effectively promote its postoperative rehabilitation, reduce the incidence of complications, improve the quality of life, and is worthy of clinical promotion.
