急性等容血液稀释对大鼠血糖、电解质及肝肾功的影响实验研究

摘要 目的:探讨两种晶体液对急性等容血液稀释大鼠血糖、电解质及肝肾功的影响,为钠钾钙镁葡萄糖注射液(乐加)安全用于急性等容血液稀释提供理论指导。方法:选择雄性SD大鼠24只,随机分成三组,每组8例:乐加组(G组)、林格氏液组(R组)和对照组(C组)。G组和R组行急性等容血液稀释,C组不做血液稀释,血液稀释组放血量是1ml/100mg,放血的同时输入3倍放血量的稀释液,血液稀释结束后继续以15ml(kg·h)速度泵注稀释液2h,于血液稀释前和血液稀释后30min监测血糖和电解质,于血液稀释前和血液稀释后2h监测肝肾功指标。结果:各组组间组内血液稀释前后电解质(K^+、Na^+、Ca^2+、Cl^-)变化比较无统计学意义(P>0.05)。与T0时间点相比,G组血液稀释前后血糖升高有统计学意义(P<0.05),各组间T1时血糖比较差异有统计学意义(P<0.05)。各组间ALT、AST、UREA、CREA的变化比较差异无统计学意义(P>0.05);R组组内AST前后变化比较差异有统计学意义(P<0.05)。结论:钠钾钙镁葡萄糖注射液和林格氏液在用于急性等容血液稀释时,在维持电解质平衡和对肝肾功影响方面相当,但钠钾钙镁葡萄糖注射液会引起血糖轻度升高。 Objective:To investigate the effects of two kinds of crystal solutions on blood glucose,electrolytes,and liver and kidney function in rats with acute normovolemic hemodilution,and to provide theoretical guidance for the safe application of sodium potassium calcium magnesium glucose injection in acute normovolemic hemodilution.Methods:24 male SD rats were randomly divided into three groups,each group of 8 cases:Sodium potassium calcium magnesium glucose injection group(G group),Ringer's solution group(R group)and Control group(C group).Acute normovolemic hemodilution was performed in G group and R group,C group was not treated with hemodilution,1ml/100mg blood was withdrawn,3 times replacement solutions was given simultaneously.After the end of the hemodilution,the replacement solutions was continued to be pumped at a rate of 15ml/kg/h for 2h.Blood glucose and electrolytes were monitored before and after the hemodilution for 30min,and liver and kidney function indicators were monitored before and 2h after the hemodilution.Results:Each group has no statistical significance in the electrolyte(K^+,Na^+,Ca^2+,Cl^-)changes(P>0.05).Compared with T 0,blood glucose was statistically significant in G group before and after the hemodilution(P<0.05).At T 1,there were statistically significant in blood glucose between the groups(P<0.05).There were no statistical significance in ALT,AST,UREA,CREA,between each group(P>0.05).The AST changes was statistically significant in R group(P<0.05).Conclusion:Sodium potassium calcium magnesium glucose injection has the equivalent effects on liver and kidney function and the maintenance electrolyte for acute normovolemic hemodilution as Ringer's solution,but sodium potassium calcium magnesium glucose injection cause a moderately elevated in blood glucose.The clinical application of sodium potassium calcium magnesium glucose injection in acute normovolemic hemodilution needs further study.
