冻干重组人脑利钠肽联合培哚普利治疗难治性心力衰竭患者的疗效及对心功能的影响

Therapeutic effect of lyophilized recombinant human brain natriuretic peptide combined with perindopril on refractory heart failure and its effect on cardiac function

目的探究冻干重组人脑利钠肽联合培哚普利治疗难治性心力衰竭(RHF)患者的疗效及对心功能的影响。方法选取84例RHF患者,采用随机数字表分为两组,各42例。控制组予以培哚普利治疗,试验组在控制组基础上予以冻干重组人脑利钠肽治疗。对比两组治疗效果及治疗前、治疗8周后血清氨基末端脑钠肽前体(NT-proBNP)、C反应蛋白(CRP)水平、每搏量(SV)、心脏指数(CI)、左心室射血分数(LVEF)。结果与控制组治疗总有效率73.81%(31/42)对比,试验组的总有效率90.48%(38/42)较高(P<0.05);治疗8周后两组血清NT-proBNP、CRP水平均降低,试验组血清NT-proBNP、CRP水平较控制组低(P<0.05);治疗后SV、CI、LVEF均升高,试验组升高幅度较控制组明显(P<0.05)。结论冻干重组人脑利钠肽联合培哚普利能提高RHF治疗效果,调节血清NT-proBNP、CRP水平,增强患者心功能。

Objective To investigate the therapeutic effect of lyophilized recombinant human brain natriuretic peptide combined with perindopril on refractory heart failure(RHF)and its effect on cardiac function. Methods 84 patients with RHF in our hospital were selected and randomly divided into two groups,with 42 cases in each group.Control group were treated with perindopril,and experimental group were treated with lyophilized recombinant human brain natriuretic peptide on the basis of the control group.And then,therapeutic effects as well as serum levels of N terminal proB type natriuretic peptide(NT-proBNP),C-reactive protein(CRP),stroke volume(SV),cardiac index(CI) and left ventricular ejection fraction(LVEF) were compared between the two groups before treatment and 8 weeks after treatment. Results The total effective rate of treatment in the control group and the experimental group was 73.81%(31/42) and 90.48%(38/42) respectively,that in the experimental group was higher( P <0.05).After 8 weeks of treatment,the levels of NT-proBNP and CRP in the two groups reduced,and those of the experimental group were lower than those of the control group( P <0.05).After treatment,the levels of SV,CI and LVEF all increased in both groups,and those of the experimental group increased more significantly( P <0.05). Conclusion Lyophilized recombinant human brain natriuretic peptide combined with perindopril can improve the therapeutic effect of RHF,regulate the levels of NT-proBNP and CRP in serum,and enhance the cardiac function of patients.
