原发性肝癌患者内毒素与肠道菌群变化的相关性研究

Endotoxin and intestinal microflora in patients with hepatocellular carcinoma

目的探讨原发性肝癌患者血液中的内毒素及粪便中肠道菌群的变化与原发性肝癌的相关性。方法采用实时荧光定量PCR方法检测双歧杆菌、乳酸杆菌、肠球菌、大肠杆菌在原发性肝癌组和正常组中的表达差异。检测内毒素在原发性肝癌组和正常组中的差别。结果大肠杆菌和肠球菌在原发性肝癌组明显增加和正常健康组比较差异有统计学意义(P <0. 05),双歧杆菌及乳酸杆菌在原发性肝癌组明显减少,和正常健康组比较差异有统计学意义(P<0.05)。原发性肝癌组血清中内毒素含量明显高于正常健康对照组,差异有统计学意义(P<O.05)。双歧杆菌、乳酸杆菌在原发性肝癌组增加,肠球菌、大肠杆菌在原发性肝癌组之间减少,内毒素含量变化在原发性肝癌组与肿瘤数量、肿瘤直径呈显著正相关。结论原发性肝癌患者中双歧杆菌、乳酸杆菌减少,肠球菌、大肠杆菌增加,这几种菌属的变化和内毒素的增加可能与原发性肝癌的发生有相关性。

Objective To study the changes of endotoxin in blood, Escherichia coli, lactobacillus, bifidobacterium, enterococci in feces of HCC patients. Methods Real-time fluorescence quantitative PCR was used to detect the difference between HCC group and normal group in the expression in bifidobacterium, lactobacillus, enterococcus and Escherichia coli, and the difterence of endotoxin level. Results The expression levels of Escherichia coli and enterococcus in HCC group were significantly higher than those in normal control group ( P < 0.05 ). Bifidobacterium and lactobacillus in primary liver cancer group significantly decreased compared with the normal group ( P < 0. 05 ). The serum endotoxin level in HCC group was significantly higher than that in normal control group ( P < 0.05 ). Bifidobacterium and lactobacillus increased in HCC group. Enterococcus and escherichia coli decreased in HCC group. There was a significant positive correlation between endotoxin content and tumor number and tumor diameter in HCC patients. Conclusion Bifidobacterium, lactobacillus decreased, enterococcus and Escherichia coli increased in HCC patients. These changes along with the increase of endotoxin may relate to the development of HCC.
