我国慢病一体化管理的现状与思考

Current situation and thinking of integrated management of chronic diseases in China

基于改良后的Rainbow模型,明确慢病一体化管理的概念与内涵,并从宏观、中观、微观三个维度入手,对目前我国的慢病一体化管理模式进行梳理,归纳出我国在慢病一体化管理方面存在制度体系不健全、机构协作不够、专业协同不足、服务环节衔接不紧密等问题。提出从制度一体化、组织与机构一体化、专业一体化、服务一体化四个方面着手,提高我国的慢病一体化管理水平。

Based on the improved Rainbow model,this study clarified the concept and connotation of integrated management of chronic diseases, combed the current integrated management model of chronic diseases in China from three dimensions:macro,meso and micro,and concluded that there are some problems in integrated management of chronic diseases in China,such as imperfect system,insufficient institutional collaboration,insufficient professional collaboration and inadequate service links and so on. It was proposed to improve the integrated management of chronic diseases in China from four aspects: system integration, organization and organization integration,professional integration and service integration.
