摘要 目的通过改进生化分析仪的系统清洗程序,逐一排除导致脂肪酶检测结果假性增高的因素,最终保证脂肪酶检测结果的准确性。方法将AU480生化分析仪所有检测项目进行逐一排除确定干扰项目,并将这些干扰项目的R1和R2试剂分别作为样本进行脂肪酶检测,进一步筛选出试剂中哪种成分对脂肪酶有干扰,再进行比色杯、搅拌棒的交叉污染排除实验,最后通过交叉污染实验对检测项目的加样顺序及仪器参数进行调整。结果载脂蛋白A-I、载脂蛋白B、总胆固醇对脂肪酶检测结果没有显著干扰,高密度脂蛋白胆固醇、低密度脂蛋白胆固醇、甘油三酯对脂肪酶检测结果有显著干扰。结论全自动生化分析仪使用过程中,存在交叉污染,为了保证检测质量,应做好室内质控、仪器的维护保养,随时观察检测项目结果与临床疾病是否相符,当检测结果与临床疾病不符时主动找原因并进行复检,并与临床做好沟通。 Objective To ensure the accuracy of lipase detection results by improving the system cleaning procedure of the biochemical analyzer and eliminating the factors leading to the false increase of the lipase detection results one by one. Methods All the detection items of AU480 biochemical analyzer were excluded to determine the interference items one by one, and the R1 and R2 reagents of these interference items were used as samples to carry out lipase detection to further screen out which components in the reagent interfered with lipase. After that, the cross-contamination elimination experiment of cuvette and stirring rod was carried out. Finally, the sequence of adding samples and instrument parameters of the detection items were adjusted through the cross-contamination experiment. Results Apolipoprotein A-I, apolipoprotein B and total cholesterol did not significantly interfere with lipase detection results. High-density lipoprotein cholesterol, low-density lipoprotein cholesterol and triglyceride significantly interfered with lipase detection results. Conclusion There is cross contamination in the process of using automatic biochemical analyzer. In order to ensure the quality of testing, it is supposed to do indoor quality control and observe whether the test results are consistent with clinical diseases at any time. When the test results do not match the clinical disease, it is necessary to take the initiative to find the causes and conduct a re-examination, and communicate with the clinic.
