摘要
目的:探讨血清miR-21在多囊卵巢综合征(PCOS)患者中的表达及其临床意义。方法:选取我院收治的150例PCOS患者(PCOS组)和50例健康女性(对照组),PCOS患者根据胰岛素抵抗指数(HOMA-IR)分为PCOS胰岛素抵抗组( n =96,HOMA-IR≥2.57)和PCOS非胰岛素抵抗组( n =54,HOMA-IR<2.57)。采用实时定量PCR(qRT-PCR)法检测各组血清miR-21表达水平。应用受试者工作特征(ROC)曲线分析血清miR-21表达水平对PCOS的诊断价值。Pearson相关分析血清miR-21与HOMA-IR的相关性。结果: PCOS组血清miR-21表达水平(1.76±0.38 VS 0.50±0.07)明显高于对照组( P <0.01)。胰岛素抵抗组血清miR-21表达水平(2.65±0.80 VS 0.55±0.08, 0.50±0.07)明显高于非胰岛素抵抗组和对照组( P <0.01)。ROC曲线显示,当血清miR-21表达水平取最佳截值为1.28时,其诊断PCOS的AUC为0.795(95%CI:0.734~0.854),敏感度和特异度为76.4%和74.8%。相关分析显示,PCOS患者血清miR-21与HOMA-IR呈正相关( r =0.817, P <0.01)。结论:血清miR-21在PCOS患者中高表达,且与胰岛素抵抗有关,对PCOS诊断具有一定价值。
Objective: To investigate the expression of serum miR-21 in patients with polycystic ovary syndrome (PCOS) and its clinical significance. Methods: 150 PCOS patients (PCOS group) and 50 healthy women (control group) admitted to the First Affiliated Hospital of Fujian Medical University were selected. PCOS patients were divided into PCOS insulin resistance group ( n =96,HOMA-IR≥2.57) and PCOS non-insulin resistance group ( n =54,HOMA-IR<2.57) according to the insulin resistance index (HOMA-IR). Real-time quantitative PCR (qRT-PCR) was used to detect the expression level of serum miR-21 in each group. The diagnostic value of serum miR-21 expression level in PCOS was analyzed by ROC curve. Pearson correlation analysis was used to analyze the correlation between serum miR-21 and HOMA-IR. Results: The expression level of serum miR-21 in PCOS group (1.76±0.38 VS 0.50±0.07) was significantly higher than that in control group ( P <0.01).The expression level of serum miR-21 in insulin resistance group (2.65±0.80 VS 0.55±0.08, 0.50±0.07) was significantly higher than that in non-insulin resistance group and control group ( P <0.01).ROC curve showed that when the optimal cut-off value of serum miR-21 expression was 1.28, the AUC for diagnosis of PCOS was 0.795 (95%CI:0.734~0.854),the sensitivity and specificity were 76.4% and 74.8%.The correlation analysis showed that serum miR-21 was positively correlated with HOMA-IR in PCOS patients ( r =0.817, P <0.01). Conclusion: Serum miR-21 is highly expressed in patients with PCOS and is related to insulin resistance, which is valuable for the diagnosis of PCOS.
