未通过听力筛查的婴儿听力追踪随访分析 被引量:5

摘要 目的探讨未通过听力筛查的婴儿听力随访结果分析.方法选取2016年11月至2017年12月期间在嘉兴市各级医院出生并接受耳声发射(OAE)或者自动听性脑干反应(AABR)新生儿听力筛查,因二次听筛未通过转诊至本院的婴儿1126名.其中206例初诊(3月龄内)为听力异常,并进行听力学跟踪随访6个月至1年,最终以听性脑干反应(ABR)、1000Hz声导抗的结果进行确诊.结果 3月龄内206例婴儿初诊为单耳听力异常101例,双耳听力异常105例.其中轻度109例,中度49例,重度14例,极重度34例.6月龄至1岁时确诊为听力正常76例,单耳听力异常62例,双耳听力异常68例,听力异常共130例;轻度、中度、重度、极重度听力异常分别为58例、31例、8例和33例.对比初诊结果,听力异常程度无变化87例,加重或者减轻43例.结论未通过听力筛查的婴儿中,轻、中度听力异常者部分恢复正常,重度、极重度听力异常者基本无改变. Objective To explore the follow-up results of infants who failed to pass hearing screening. Methods There were 1126 infants who had failed to the second screening referral in our hospital. They were borned in various hospitals in Jiaxing city and received otoacoustic emissions or automatic auditory brainstem neonatal hearing screening from November 2016 to December 2017. Of these,206 cases were initial diagnosed(within 3 months of age)for abnormal hearing,and followed-up for 6 months to 1 year by audiology. The final diagnosis was based on the results of auditory brainstem response and 1000Hz tympanometry. Results The initial diagnosis of 206 infants within 3 months of age was 101 cases of monaural abnormal hearing and 105 cases of binaural abnormal hearing. Among them,109,49,14,and 34 cases were found to be mild,moderate,severe and profound abnormal hearing respectively. From 6 months to 1 year of age,76 cases were confirmed with normal hearing and 130 cases with abnormal hearing,including 62 cases of monaural abnormal hearing and 68 cases of binaural abnormal hearing. Among these 130 cases,mild abnormal hearing was observed in 58 cases,moderate in 31,severe in 8 cases,and profound in 33 cases. Compared with the results of the initial diagnosis,there were no changes in the degree of hearing abnormalities in 87 cases,and 43 cases were aggravated or alleviated. Conclusion Among the infants who failed to pass the hearing screening,some of the mild and moderate hearing abnormalities returned to normal, while the severe and extremely severe hearing abnormalities remained basically unchanged.
