腰痹通胶囊联合脉冲射频治疗老年慢性腰腿痛的临床研究

Clinical Study of Yaobitong capsule combined with pulse radiofrequency in the treatment of elderly patients with chronic lumbago and leg pain

目的:探讨应用腰痹通胶囊联合脉冲射频治疗老年慢性腰腿痛,观察其临床效果,并检测相关炎性因子水平,分析作用机制。方法: 120例老年慢性腰腿痛患者,数表法随机分为对照组和研究组,两组各60例,对照组应用脉冲射频治疗,研究组同时应用腰痹通胶囊进行治疗。评估两组患者临床疗效、应用改良日本骨科学会腰痛功能评价表(M-JOA)评估患者腰腿部功能、采用视觉模拟评分法(VAS)、ODI量表评估患者的疼痛、功能障碍和生活状况,并检测白细胞介素-6(IL-6)、IL-8、肿瘤坏死因子(TNF-α)水平。结果:研究组总有效率为98.33%,高于对照组96.67%,差异有统计学意义( P <0.05);研究组治疗后M-JOA总评分为(22.85±1.73)分,高于对照组的(20.31±1.56)分;研究组VAS评分和ODI评分分别为(2.33±0.48)分和(25.24±2.70)分,低于对照组的(3.54±0.51)分和(30.18±2.93)分,差异均具有统计学意义( P <0.05)。研究组治疗后血清IL-6、IL-8、和TNF-α分别为(32.31±4.58)pg/mL、(64.62±7.08)pg/mL和(25.02±3.65)pg/mL,均低于对照组(38.65±4.46)pg/mL、(72.38±6.90)pg/mL和(31.62±3.74)pg/mL,差异均具有统计学意义( P <0.05)。结论:应用腰痹通胶囊联合脉冲射频治疗老年慢性腰腿痛,能够显著改善患者症状或体征,减轻腰腿痛疼痛程度,下调炎性因子水平。

Objective: To explore the application of Yaobitong capsule combined with pulse radiofrequency in the treatment of the elderly with chronic low back pain,to observe its clinical effect,to evaluate the improvement of symptoms or signs,and to detect the level of related inflammatory factors,and to analyze its mechanism of action. Methods: 120 elderly patients with chronic low back pain were selected and randomly divided into the control group and the study group,60 patients in each group.The control group was treated with pulsed radiofrequency ablation,study group treated with YaoBitong capsule and pulsed radiofrequency ablation.Evaluated the clinical efficacy,evaluated the waist and leg function of the patients using the modified Japanese Orthopaedic Association Low Back Pain Function Evaluation Form(M-JOA),assessed the patient s pain,dysfunction and life using visual analogue scale(VAS) and ODI scales.And the levels of interleukin-6(IL-6),IL-8,tumor necrosis factor(TNF-α) were measured. Results: The total effective rate of the study group was 98.33%,which was higher than that of the control group 96.67%,the difference was statistically significant( P <0.05).M-JOA total score of the study group after treatment was(22.85±1.73) points,which was higher than that of the control group(20.31±1.56) points,VAS score and ODI score of the study group were(2.33±0.48) points and(25.24±2.70) points,which was lower than that of the control group(3.54±0.51) points and(30.18±2.93) points,the differences were statistically significant( P <0.05).Serum IL-6,IL-8,and TNF-α in the study group were(32.31±4.58) pg/mL,(64.62±7.08) pg/mL and(25.02±3.65) pg/mL, lower than that of the control group(38.65±4.46) pg/mL,(72.38±6.90) pg/mL and(31.62±3.74) pg/mL,the differences were statistically significant( P <0.05). Conclusion: Application of YaoBitong Capsule combined with pulse radiofrequency in the treatment of elderly patients with chronic low back pain has significant clinical effects,which can significantly improve the symptoms or signs of the patients,reduce the pain of low back and leg pain,and down-regulate the level of inflammatory factors.
