摘要
分析了医院健康联合体慢性病管理的过程和数据需求,结合不同慢性病的管理指南,从需求分析、系统设计、建设难点3个方面,利用软件工程学方法,探讨医院健康联合体信息平台的建设,对系统建设中存在的难点问题进行分析,并给出解决办法。通过平台建设,实现医院健康联合体成员单位慢性病管理流程标准化、规范化。
With a combination of the chronic diseases management guidelines, this paper analyzes the process and data needs for chronic disease management in hospital consortium. From the aspects of demand analysis, system design and construction difficulties, using software engineering methods and analyzing difficult problems, it explores the solutions of health information platform construction for hospital consortium. Through the platform construction, the standardized chronic disease management process among the member hospitals will be realized.
