急性ST段抬高型心肌梗死患者住院费用影响因素分析

Study on influential factors of hospitalization costs of acute ST-segment elevation myocardial infarction inpatients

目的对某院2015-2017年急性ST段抬高型心肌梗死患者住院费用进行研究,分析住院费用构成,探讨各相关因素对住院费用的影响,为控制住院费用过快增长提供依据。方法收集符合条件的住院病例共1010例,采用描述性分析、相关分析、秩和检验和多重线性回归等方法分析住院费用变化趋势、构成情况及其影响因素。结果患者人均住院费用逐年下降,材料费占比最高(54.62%),住院费用主要受住院时间影响。医疗付款方式、入院途径、离院方式、抢救次数、手术类型、植入支架个数等均对住院费用有影响。结论该院医疗费用控制取得进步。优化诊疗流程,缩短平均住院时间,规范诊疗行为,严格把控住院和手术指征等措施均有助于合理控制医疗费用,降低患者疾病负担。

ObjectiveTo study the hospitalization expenses of patients with acute ST-segment elevation myocardial infarction in a hospital from 2015 to 2017,analyze the composition of hospitalization expenses,and explore the impact of various related factors on hospitalization expenses,and provide a basis for controlling the excessive growth of hospitalization expenses.MethodsA total of 1 010 eligible hospitalized cases were collected.Descriptive analysis,correlation analysis,rank sum test and multiple linear regression were used to analyze the trend,composition and influencing factors of hospitalization expenses.ResultsThe per capita hospitalization cost of patients decreased year by year,and the proportion of materials was the highest(54.62%).The hospitalization expenses were mainly affected by the hospitalization time.The results showed that the medical payment method,admission route,way of leaving the hospital,number of rescues,type of surgery,number of implants,etc.all had an impact on hospitalization expenses.ConclusionMedical cost control in the hospital has progressed.Optimizing the diagnosis and treatment process,shortening the average hospitalization time,standardizing the diagnosis and treatment behavior,and strictly controlling the hospitalization and surgical indications all help to rationally control medical expenses and reduce the patient′s disease burden.
