尿NGAL在新生儿窒息后急性肾损伤诊治中的应用 被引量:2

摘要 目的:探讨尿中性粒细胞明胶酶相关脂质运载蛋白(NGAL)在新生儿窒息后急性肾损伤诊治中的应用。方法:择取2018年2月至2019年5月期间惠东县人民医院收治的在围生期发生窒息的足月新生儿共120例为研究对象,根据其出生后7d内是否发生了急性肾损伤为依据分为急性肾损伤组和非急性肾损伤组,同时急性肾损伤组中再依据新生儿病情严重程度的分为危险期组、损伤期组和衰竭期组,在新生儿出后24h内进行尿NGAL、BUN及Scr水平检测,并进行组间比较。结果:本研究120例新生儿发生急性肾损伤共42例,发生率为35.00%,即非急性肾损伤组78例新生儿,急性肾损伤组42例新生儿,其中危险期组20例、损伤期组12例和衰竭期组10例。急性肾损伤组新生儿24h内尿NGAL显著高于非急性肾损伤组,组间比较,差异具有统计学意义(P<0.05);组间BUN及Scr水平比较,差异无统计学意义(P>0.05);衰竭期新生儿24h内尿NGAL水平高于危险期和损伤期新生儿,且损伤期新生儿24h内尿NGAL水平高于危险期,组间比较,差异具有统计学意义(P<0.05),而三组间BUN及Scr水平差异无统计学意义(P>0.05)。结论:检测尿NGAL水平能够有效预测窒息新生儿急性肾损伤发生风险,且有利于医生及时了解患儿病情进展,为其临床治疗方案的制定和实施提供了有效依据。
