呼吸道病毒感染对哮喘患儿气道重塑与炎症反应程度的影响

Effects of respiratory viral infection on airway remodeling and degree of inflammatory reaction in children with asthma

目的:探讨呼吸道病毒感染对哮喘患儿气道重塑与炎症反应程度的影响。方法:按照随机数字表法抽取哮喘合并呼吸道病毒感染患儿43例,将其作为研究组;同法抽取同期收治的单纯哮喘患儿43例,将其作为对照组;均给予两组高分辨率CT检测肺部,以及血清指标检测,对比两组检测结果。结果:研究组右上叶尖段(RB1)、左下叶后基底段(LB10)CT气道重塑管腔面积(LA)、管壁面积(WA)、支气管总面积(TA)的参数水平均低于对照组,差异有统计学意义(P<0.05);研究组转化生长因子β1(TGF-β1)、Smad3、Ⅰ型胶原(PINP)、Ⅲ型胶原(PⅢNP),白细胞介素-4(IL-4)、白细胞介素-5(IL-5)、肿瘤坏死因子-α(TNF-α)等指标水平均高于对照组,差异有统计学意义(P<0.05)。结论:哮喘患儿若合并呼吸道病毒感染,极易加重气道重塑、炎症反应程度,临床诊疗应重视哮喘患儿呼吸道病毒感染的可能性,并及时采用抗病毒治疗。

Objective:To study effects of respiratory viral infection on airway remodeling and degree of inflammatory reaction in children with asthma.Methods:According to the random number table method,43 children with asthma complicated with respiratory viral infection were used as the study group,while 43 children with simple asthma in the same period were set as the control group.The lungs were scanned by using the high-resolution CT,and the serum indicators were tested.Then,the results of the two groups were compared.Results:The CT airway remodeling lumen area(LA),wall area(WA)and total bronchial area(TA)of the right upper apical segment(RB1)and the left lower basal segment(LB10)and other target organs of the study group were lower than those of the control group,and the differences were statistically significant(P<0.05).The study group was transformed into growth factorβ1(TGF-β1),Smad3,type I collagen(PINP),type III collagen(PⅢNP),interleukin-4(IL-4),interleukin-5(IL-5)and tumor necrosis factor-α(TNF-α)of the study group were higher than those of the control group,and the differences were statistically significant(P<0.05).Conclusions:Asthma complicated with respiratory viral infection in the children can easily aggravate the degree of airway remodeling and inflammatory reaction.Therefore,clinical diagnosis and treatment should pay attention to the possibility of respiratory viral infection,and timely use the antiviral therapy.
