摘要
目的探讨血管内皮生长因子(VEGF)和微血管密度(MVD)在食管胃结合部腺癌(AEG)中的表达及临床意义。方法选取158例AEG患者的AEG组织及癌旁正常胃黏膜组织,另选取130例胃黏膜上皮内瘤变患者的低级别上皮内瘤变组织51例和高级别上皮内瘤变组织79例,采用免疫组织化学法和蛋白质印迹法(Westernblot)检测VEGF表达情况和MVD,并对VEGF表达和MVD与AEG患者临床特征的关系进行分析。结果癌旁正常胃黏膜组织中VEGF表达情况和MVD均低于低级别上皮内瘤变组织、高级别上皮内瘤变组织和AEG组织;AEG组织中VEGF表达情况和MVD均高于低级别上皮内瘤变组织和高级别上皮内瘤变组织,差异均有统计学意义(P﹤0.05)。中低分化、弥散型及混合型、浸润肌层和有淋巴结转移AEG患者AEG组织中VEGF表达情况高于高分化、肠型、浸润黏膜层和无淋巴转移的患者,差异均有统计学意义(P﹤0.05);中低分化、浸润肌层和有淋巴转移AEG患者的AEG组织中MVD值分别高于高分化、浸润黏膜层和无淋巴结转移的患者,差异均有统计学意义(P﹤0.05)。AEG组织中,VEGF表达水平与MVD值呈正相关(r=0.491,P﹤0.05)。结论AEG组织中VEGF和MVD表达升高,并且与AEG的发生发展密切相关。
Objective To investigate the expression of vascular endothelial growth factor(VEGF)and microvessel density(MVD)in the adenocarcinoma of esophagogastric junction(AEG)and their clinical significance.Method AEG tissues and paired adjacent normal gastric mucosa were selected from 158 patients with AEG,and gastric mucosa from 130 patients with benign non-invasive gastric intraepithelial neoplasia,including 51 samples from low-grade dysplasia and 79 samples from high-grade dysplasia.The expression of VEGF protein and MVD in gastric mucosa was detected by immunohistochemistry and Western blot,and their relationship with clinical pathological features of the AEG patients were analyzed.Result The expression of VEGF and MVD was lower in adjacent normal gastric mucosa tissues than that in tissues from either patients with AEG or patients with low-grade gastric intraepithelial neoplasia or high-grade gastric intraepithelial neoplasia,and the expression of VEGF and MVD in the AEG patients were higher than that in patients with neither low-grade or high-grade gastric intraepithelial neoplasia the differences were statistically significant(P<0.05).The expression of VEGF in AEG tissues from patients with moderate and poorly differentiated,diffuse and mixed type,muscle layer infiltration and lymph node metastasis was higher than that in patients with well differentiated,intestinal type,mucosa layer infiltration and no lymph node metastasis and the differences were statistically significant(P<0.05),and the expression of MVD was significant higher in AEG patients with moderate and poorly differentiated,muscle layer infiltration and lymph node metastasis than that in patients with well differentiated,mucosa layer infiltration and no lymph node metastasis(P<0.05).The expression level of VEGF was positively correlated with the expression of MVD in AEG tissues(r=0.491,P<0.05).Conclusion VEGF and MVD are highly expressed in AEG samples and may be closely associated with the occurrence and development of the AEG.
