早期康复治疗对老年重症肺炎机械通气患者并发症及预后的影响

Effect of early rehabilitation treatment on complications and prognosis of senile patients with mechanical ventilation of severe pneumonia

目的分析早期康复治疗对老年重症肺炎机械通气患者并发症及预后的影响。方法选取2014年1月~2018年5月我院收治的老年重症肺炎142例患者作为研究对象,应用抽签法随机分为研究组和对照组,每组各71例。对照组采用常规方案进行治疗,研究组采用早期康复进行治疗;比较两组治疗后的并发症、预后情况。结果研究组和对照组治疗后并发症总发生率分别为88.73%和36.62%,差异有统计学意义(P<0.05)。研究组治疗后预后谵妄持续时间、机械通气时间、重症加强护理病房(ICU)住院时间明显短于对照组,30d病死率、拔管失败率、气管切开率明显低于对照组,差异均有统计学意义(P<0.05)。结论对老年重症肺炎机械通气患者行早期康复治疗,能降低并发症的发生率,且有利于改善患者预后,值得临床推广。

Objective To analyze the effect of early rehabilitation treatment on the complications and prognosis of senile patients with severe pneumonia mechanical ventilation. Methods A total of 142 senile patients with severe pneumonia admitted in our hospital from January 2014 to May 2018 were selected as the study subjects. They were randomly divided into the study group and the control group by drawing lots, 71 cases in each group. The control group was treated with routine treatment, while the study group was treated with early rehabilitation treatment. The complications and prognosis of the two groups after treatment were compared. Results The total incidence of complications in the study group and the control group were 88.73% and 36.62% respectively, with significant difference (P<0.05). The duration of prognosis delirium, duration of mechanical ventilation, hospitalization time in intensive care unit (ICU) in the study group were significantly shorter than those in the control group, and the mortality, extubation failure rate and tracheotomy rate in the 30 days were significantly lower than those in the control group (P<0.05). Conclusion Early rehabilitation treatment for senile patients with severe pneumonia mechanical ventilation can reduce the incidence of complications, improve the prognosis of patients, it is worth promoting in clinical practice.
