硝酸甘油联合硫酸镁治疗妊娠期高血压的效果及对妊娠结局的影响

Effect of Nitroglycerin Combined with Magnesium Sulfate on Gestational Hypertension and Its Effect on Pregnancy Outcome

目的:研究妊娠期高血压患者使用硝酸甘油联合硫酸镁治疗效果及对妊娠结局的影响。方法:选择笔者所在医院2016年5月-2018年5月入院诊治的84例妊娠期高血压产妇,随机分为观察组和对照组,各42例。对照组使用硫酸镁治疗,观察组使用硝酸甘油联合硫酸镁治疗。观察治疗前及治疗1周后,两组血压(收缩压、舒张压)、一氧化氮(NO)、血清内皮素(ET)变化情况,统计妊娠结局(分娩方式、妊娠并发症),记录不良反应。结果:观察组治疗后收缩压、舒张压均低于对照组(P<0.05)。观察组治疗后NO、ET水平较对照组均明显改善(P<0.05)。观察组治疗后顺产率高于对照组(P<0.05)。观察组治疗后妊娠并发症发生率低于对照组(P<0.05)。结论:硝酸甘油联合硫酸镁对妊娠期高血压患者血压控制效果较好,并在一定程度上改善血管内皮功能及妊娠结局。

Objective:To study the effect of Nitroglycerin combined with Magnesium Sulfate on pregnancy outcome in patients with gestational hypertension. Method:A total of 84 cases with gestational hypertension who were admitted and treated in the hospital of the author from May 2016 to May 2018 were randomly divided into the observation group and the control group,and 42 cases in the each group.The control group was treated with Magnesium Sulfate,while the observation group was treated with Nitroglycerin combined with Magnesium Sulfate.The changes of blood pressure(systolic blood pressure,diastolic blood pressure), nitric oxide(NO) and endothelin(ET) in the two groups were observed before and 1 week after treatment.The pregnancy outcomes(mode of delivery,complications of pregnancy) were counted and adverse reactions were recorded.Result:After treatment,the systolic and diastolic blood pressure in the observation group were lower than those of the control group(P<0.05),and the levels of NO and ET in the observation group were significantly improved compared with those of the control group(P<0.05).The rate of spontaneous delivery in the observation group was higher than that of the control group(P<0.05),and the incidence of pregnancy complications in the observation group was lower than that of the control group(P<0.05).Conclusion:Nitroglycerin combined with Magnesium Sulfate has a good effect on blood pressure control in patients with gestational hypertension,and improves vascular endothelial function and pregnancy outcome to a certain extent.
