经肌间隙椎弓根螺钉内固定术治疗青少年无神经损伤性胸腰椎骨折的效果及对细胞因子的影响

Effect of transmuscular space pedicle screw internal fixation on thoracolumbar fractures without nerve injury in adolescents and its effect on cytokines

摘要 目的探讨经肌间隙椎弓根螺钉内固定术治疗青少年无神经损伤性胸腰椎骨折的效果及对细胞因子的影响。方法将本院接收的52例无神经损伤的胸腰椎骨折患者采用电脑随机数列分组法分为对照组(开放式椎弓根螺钉内固定术)与观察组(经肌间隙椎弓根螺钉内固定术),各26例。比较两组的治疗效果。结果观察组切口长度与手术时间均短于对照组,术中出血量少于对照组(P<0.05)。术后1周,两组VAS、T-AOC水平评分均降低,MCP-1水平均升高,观察组均优于对照组(P<0.05);术后3个月,两组Cobb角均减小,伤椎前缘高度比值均增大(P<0.05)。结论经肌间隙椎弓根螺钉内固定术治疗青少年无神经损伤性胸腰椎骨折创伤小,患者术后疼痛小,康复快。 Objective To evaluate the effect of transmuscular space pedicle screw internal fixation on thoracolumbar fractures without nerve injury in adolescents and its effect on cytokines. Methods A total of 52 patients with thoracolumbar fractures without nerve injury who were treated in our hospital were divided into control group (open pedicle screw fixation) and observation group (transmuscular space pedicle screw fixation) by computer random sequence grouping method, with 26 cases in each group. The therapeutic effects of the two groups were compared. Results The incision length and operation time of the observation group were shorter than those of the control group, and the amount of bleeding during operation was less than that of the control group (P<0.05). At 1 week after operation, the VAS scores and T-AOC levels in both groups decreased, MCP-1 levels increased, and those in the observation group were better than the control group (P<0.05);at 3 months after operation, the Cobb angle of both groups decreased, and the ratio of anterior height of injured vertebrae increased (P<0.05). Conclusion Transmuscular space pedicle screw fixation in the treatment of thoracolumbar fractures without nerve injury in adolescents has small trauma, less postoperative pain and quick recovery.
