超声乳化人工晶体植入联合不同术式治疗急性闭角型青光眼合并白内障的临床效果

Clinical effect of phacoemulsification and intraocular lens implantation combined with different surgical methods in the treatment of acute primary angle-closure glaucoma with cataract

目的探讨超声乳化人工晶体植入联合不同术式治疗急性闭角型青光眼(APACG)合并白内障的临床效果。方法选取我院收治的54例(54只患眼)APACG合并白内障患者为研究对象,将其随机分为观察组(超声乳化人工晶体植入+房角分离术)和对照组(超声乳化人工晶体植入+小梁切除术),比较两组患者的疗效。结果术后6个月,两组前房深度、视力、眼压均较术前明显改善,且观察组前房深度优于对照组(P<0.05)。术后6个月,两组FIB、APTT均显著改善,且观察组优于对照组(P<0.05)。观察组并发症总发生率低于对照组(P<0.05)。结论超声乳化人工晶体植入联合房角分离术或小梁切除术治疗APACG合并白内障均可行,临床应结合患者个体情况选择手术方式。

Objective To investigate the clinical effects of pha coemulsification and intraocular lens implantation combined with different surgical methods in the treatment of acute primary angle-closure glaucoma (APACG) with cataract. Methods Fifty-four cases (54 affected eyes) of APACG with cataract in our hospital were selected as the research objects and randomly divided into observation group (phacoemulsification and intraocular lens implantation goniosynechialysis) and control group (phacoemulsification and intraocular lens implantation+trabeculectomy). The treatment effects of the two groups were compared. Results Six months after operation, the anterior chamber depth, visual acuity and intraocular pressure in the two groups significantly improved, and the anterior chamber depth in the observation group was better than that in the control group (P<0.05). Six months after operation, the FIB and APTT in both groups significantly improved, and those in the observation group were better than the control group (P<0.05). The total incidence of complications in the observation group was lower than that in the control group (P<0.05). Conclusion Phacoemulsification and intraocular lens implantation combined with goniosynechialysis or trabeculectomy for APACG with cataract are feasible, and the operation method should be selected according to the individual condition of the patient.
