摘要
目的研究宫颈癌腹腔镜手术最后的结果与其影响预后的各类因素。方法选出我院在2015 年1 月-2018 年12 月采用开腹手术的32例宫颈癌患者,将其作为A 组,另选出在此基础之上采用腹腔镜手术的32 例宫颈癌患者,将其作为B 组,观察比较其结果。结果 B 组患者在治疗后其淋巴结清扫总数、血红蛋白都优于A 组患者(P < 0.05),而其白细胞两组间差异无统计学意义(P > 0.05);有脉管瘤栓、有盆腔淋巴结转移、深肌层浸润、多于Ⅰ b2 期患者的总复发率多于无脉管瘤栓、无盆腔淋巴结转移、浅肌层浸润、少于Ⅰ b2 期患者,五年中总生存率更少(P < 0.05);对宫颈癌腹腔镜手术预后带来影响的各类因素主要就包括了脉管瘤栓、盆腔淋巴结转移、肌层浸润深度、临床分期(P < 0.05)。结论对于宫颈癌患者来说,在运用腹腔镜手术后,能够最大限度地提升其治疗最后的效果与质量,同时,临床分期、盆腔淋巴结转移等相关情况极有可能对预后带来相应的影响。
Objective To study the final outcome of laparoscopic surgery for cervical cancer and its prognostic factors. Methods 32 patients with cervical cancer who underwent laparotomy in our hospital from January 2015 to December 2018 were selected as group A, and 32 patients with cervical cancer who underwent laparoscopic surgery on this basis were selected as group B to observe and compare the results. Results The total number of lymph node dissection and hemoglobin in group B were better than those in group A (P<0.05), and the white blood cell count did not have statistical difference (P>0.05). The total recurrence rate of patients with vascular emboli, pelvic lymph node metastasis, deep muscle infiltration and more than stage I B2 was higher than that of patients without vascular emboli, pelvic lymph node metastasis, superficial muscle infiltration and less than stage I b2, and the overall survival rate in five years was lower (P< 0.05). The main factors affecting the outcome were angioma thrombus, pelvic lymph node metastasis, myometrial invasion depth and clinical stage (P<0.05). Conclusion For patients with cervical cancer, laparoscopic surgery can maximize the final effect and quality of treatment. At the same time, clinical staging, pelvic lymph node metastasis and other related conditions are likely to have a corresponding impact on the prognosis.
