Logistic regression analysis of risk factors influencing the recurrence of chronic rhinosinusitis with nasal polyps after operation

目的分析慢性鼻-鼻窦炎伴鼻息肉(CRSwNP)术后复发的危险因素。方法回顾性分析86例CRSwNP患者的临床资料,根据是否复发将患者分为A组(复发,29例)与B组(未复发,57例)。分析复发的危险因素。结果 A组抽烟、变应性鼻炎、支气管哮喘、鼻中隔偏曲、OMC评分2分、流涕评分>5分、Lund-Mackay评分>10分、外周血Eos占比>5.3%、外周血Neu占比<55.5%的患者构成比均高于B组(P<0.05),多因素Logistic回归分析显示上述指标均为影响CRSwNP术后复发的独立危险因素(P<0.05)。结论抽烟、变应性鼻炎、支气管哮喘、鼻中隔偏曲、OMC评分2分、流涕评分>5分、Lund-Mackay>10分、外周血Eos占比>5.3%、外周血Neu占比<55.5%为影响CRSwNP患者术后复发的独立危险因素,临床应针对相应因素给予患者适当干预。

Objective To analyze the risk factors influencing the recurrence of chronic rhinosinusitis with nasal polyps (CRSwNP) after operation. Methods The clinical data of 86 patients with CRSwNP were retrospectively analyzed, the patients were divided into group A (recurrence, 29 cases) and group B (no recurrence, 57 cases) according to whether recurrence occurred. The risk factors for recurrence were analyzed. Results The proportions of patients smoking, with allergic rhinitis, bronchial asthma, deviation of nasal septum, OMC score of 2 points, runny score >5 points, Lund-Mackay score >10 points, peripheral blood eosinophil (Eos) ratio >5.3%, peripheral blood neutrophil (Neu) ratio of <55.5% were higher than those in the group B (P<0.05), and multivariate Logistic regression analysis showed that all above indicators were independent risk factors for postoperative recurrence of CRSwNP (P<0.05). Conclusion Smoking, allergic rhinitis, bronchial asthma, deviation of nasal septum, OMC score is 2 points, runny score >5 points, Lund-Mackay score >10 points, peripheral blood eosinophil (Eos) ratio >5.3%, peripheral blood neutrophil (Neu) ratio <55.5% are independent risk factors for postoperative recurrence in patients with CRSwNP, appropriate intervention should be given to patients according to relevant factors in clinic.
