奥曲肽联合不同剂量泮托拉唑治疗消化性溃疡伴消化道出血的效果及安全性

Efficacy and safety of octreotide combined with pantoprazole in the treatment of peptic ulcer with gastrointestinal bleeding

目的分析奥曲肽联合不同剂量泮托拉唑治疗消化性溃疡伴消化道出血的效果及安全性。方法选取2015年1月至2018年1月于我院就诊的80例消化性溃疡伴消化道出血患者为研究对象,根据不同用药方案将其分为对照组(40例,奥曲肽+常规剂量泮托拉唑)与观察组(40例,奥曲肽+大剂量泮托拉唑)。比较两组的治疗效果。结果观察组的治疗总有效率、止血时间、住院时间、输血量及再出血率均显著优于对照组(P<0.05)。治疗后,两组Hb、HCT、SBP、DBP及IL-2水平均升高,IL-6、IL-17、GAS及PGⅠ水平均降低,且观察组优于对照组(P<0.05)。治疗后,两组生理功能、躯体疼痛、总体健康、社会功能及精神健康评分均升高,且观察组高于对照组(P<0.05)。两组不良反应总发生率无显著差异(P>0.05)。结论奥曲肽联合大剂量泮托拉唑治疗消化性溃疡伴消化道出血效果显著,能抑制胃酸过多分泌并改善炎症反应,且不会增加不良反应,值得临床推广。

Objective To analyze the efficacy and safety of octreotide combined with pantoprazole in the treatment of peptic ulcer with gastrointestinal bleeding. Methods Eighty patients with peptic ulcer and gastrointestinal bleeding treated in our hospital from January 2015 to January 2018 were selected as the research objects and divided into control group(40 cases, octreotide+conventional-dose pantoprazole) and observation group(40 cases, octreotide+high-dose pantoprazole). The therapeutic effects of the two groups were compared. Results The total effective rate of treatment, hemostasis time,hospitalization time, blood transfusion volume and rebleeding rate of the observation group were significantly better than those of the control group(P<0.05). After treatment, the levels of Hb, HCT, SBP, DBP and IL-2 increased in both groups,while the levels of IL-6, IL-17, GAS and PGⅠ decreased, and those in the observation group were better than the control group(P <0.05). After treatment, the scores of physiological function, physical pain, general health, social function and mental health in the two groups increased, and those in the observation group were higher than the control group(P<0.05).There was no significant difference in the total incidence of adverse reactions between the two groups(P>0.05). Conclusion Octreotide combined with high-dose pantoprazole in the treatment of peptic ulcer with gastrointestinal bleeding has a significant effect, it can inhibit gastric acid secretion and improve inflammatory response, and does not increase adverse reactions, which is worthy of clinical promotion.
