摘要 目的探讨Ⅰ期全髋关节置换术治疗晚期髋关节结核的临床效果。方法选取我院2012年4月至2016年4月收治的13例晚期髋关节结核患者为研究对象,均采用前外侧入路行人工全髋关节置换术,评价术后治疗效果。结果 13例患者均随访(27.7±14.2)个月,手术切口均甲级愈合,术后假体无松动,无脱位,无髋关节结核复发。末次随访,患者的Harris评分为(87.9±4.7)分,显著高于术前的(22.8±10.6)分(P<0.05)。末次随访,患者的髋关节屈伸活动度为(90.6°±12.3°),较术前髋关节屈伸活动度(30.2°±5.1°)显著增大(P<0.05)。结论Ⅰ期全髋关节置换术治疗晚期髋关节结核的临床效果显著,其可缓解患者术后关节疼痛,改善关节功能,且无髋关节结核复发,值得临床推广应用。 Objective To explore the clinical effect of one-stage total hip arthroplasty in the treatment of advanced hip tuberculosis. Methods A total of 13 patients with advanced hip tuberculosis admitted in our hospital from April 2012 to April 2016 were selected as the study objects. All patients underwent total hip arthroplasty through anterolateral approach, and the therapeutic effects were evaluated. Results All 13 patients were followed up for (27.7依14.2) months. All the incisions healed in grade A. There were no loosening, dislocation or recurrence of hip tuberculosis after operation. At the last follow-up, the Harris score of the patients was (87.9依4.7), which was significantly higher than (22.8依10.6) of the patients before operation (P<0.05). At the last follow-up, the hip flexion-extension range of motion was (90.6毅依12.3毅), which was significantly larger than (30.2毅依5.1毅) of the patients before operation (P<0.05). Conclusion The one-stage total hip arthroplasty in the treatment of advanced hip tuberculosis has a remarkable effect, which can relieve the joint pain after operation and improve the joint function, without recurrence of hip tuberculosis. It is worthy of clinical promotion and application.
