降钙素原及C反应蛋白检测对输尿管软镜下钬激光碎石术后脓毒血症的诊断价值

Diagnostic value of procalcitonin and C-reactive protein in sepsis after ureteroscopic holmium laser lithotripsy

目的探讨降钙素原(PCT)及C反应蛋白(CRP)检测对输尿管软镜下钬激光碎石术后脓毒血症的诊断价值。方法选择我院收治的141例输尿管软镜下钬激光碎石术后发生尿路感染和尿源性脓毒血症的患者为研究对象,按照血培养结果将其分为观察组(尿源性脓毒血症,39例)和对照组(常规泌尿系统感染,102例)。比较两组患者的PCT、CRP水平,并分析二者单独及联合检测的灵敏度、特异度。结果观察组患者的PCT、CRP水平均明显高于对照组(P<0.05)。PCT与CRP联合检测的灵敏度为92.31%,特异度为93.14%,均明显高于两者单独检测的灵敏度与特异度(P<0.05)。结论 PCT和CRP均可作为尿源性脓毒血症的诊断指标,二者联合检测能够提高尿源性脓毒血症早期诊断的准确性,为临床及时救治该病患者提供理论依据。

Objective To explore the diagnostic value of procalcitonin (PCT) and C-reactive protein (CRP) in sepsis after ureteroscopic holmium laser lithotripsy. Methods A total of 141 patients with urinary tract infection and urinary sepsis after ureteroscopic holmium laser lithotripsy admitted in our hospital were selected as the study objects. According to the blood culture results, they were divided into observation group (urinary sepsis, 39 cases) and control group (routine urinary system infection, 102 cases). The levels of PCT and CRP were compared in the two groups, and the sensitivity and specificity of alone and combined detection of them were analyzed. Results The levels of PCT and CRP of the observation group were significantly higher than those of the control group (P<0.05). The sensitivity and specificity of combined detection of PCT and CRP were 92.31%, 93.14% respectively, which were significantly higher than those of alone detection of PCT and CRP (P<0.05). Conclusion Both PCT and CRP can be used as diagnostic indicators of urinary sepsis. The combined detection of PCT and CRP can greatly improve the accuracy of early diagnosis of urinary sepsis, and provide theoretical basis for timely clinical treatment.
