匹多莫德联合泼尼松治疗小儿过敏性紫癜临床研究

Clinical Study on Pidotimod Combined with Prednisolone in the Treatment of Children with Henoch-Schonlein Purpura

目的探讨匹多莫德联合泼尼松对过敏性紫癜(HSP)患儿免疫功能、炎性反应的影响。方法选取涟水县人民医院2016年7月至2018年2月收治的过敏性紫癜患儿97例,按随机数字表法分为对照组(48例)和研究组(49例)。两组患儿口服醋酸泼尼松片(每次1 mg/kg,每日1次),研究组患儿加服匹多莫德颗粒(每次0. 4 g,每日1次),连续治疗1个月。结果研究组有效率为89. 80%,显著高于对照组的58. 33%(P <0. 01);与治疗前比较,两组患儿治疗后T淋巴细胞(CD3^+,CD4^+,CD4^+/CD8^+)均显著升高,白细胞介素15(IL-15)、白细胞介素23(IL-23)、肿瘤坏死因子-α(TNF-α)水平均显著降低,且研究组上述指标改善程度均显著优于对照组(P <0. 05);对照组与研究组患儿不良反应发生率相当(8. 33%比10. 20%,χ^2=0. 101,P=0. 751> 0. 05)。结论匹多莫德联合泼尼松治疗HSP,可减轻炎性反应,增强机体免疫功能,且不增加不良反应。

Objective To investigate the effect of pidotimod combined with prednisolone on immune function and inflammatory reaction in children with Henoch - Schonlein purpura ( HSP ). Methods Totally 97 children with HSP admitted to Lianshui People's Hospital from July 2016 to February 2018 were selected and divided into the control group ( n = 48 ) and the study group ( n = 49 ) according to the random number table method. The patients in the two groups were treated with Prednisolone Tablets ( 1 mg /kg each time,once a day ), on this basis,the patients in the study group were treated with Pidotimod Granules ( 0. 4 g each time,once a day ). Both groups were treated continuously for 1 month. Results The effective rate of the study group was 89. 80%,which was significantly higher than 58. 33% of the control group ( P < 0. 01 ). Compared with those before treatment,the T lymphocyte ( CD 3 ^+,CD 4 ^+ and CD 4 ^+/CD 8 ^+) was significantly increased,while the levels of interleukin - 15 ( IL - 15 ),IL - 23,tumor necrosis factor -α( TNF -α) were significantly decreased in the two groups after treatment,and the above indexes in the study group were significantly better than those in the control group ( P < 0. 05 ). The incidence of adverse reactions in the control group was similar to that in the observation group ( 8. 33% vs. 10. 20%,χ^2 = 0. 101,P = 0. 751 > 0. 05 ). Conclusion Pidotimod combined with prednisolone in the treatment of HSP can alleviate inflammatory reaction and enhance immune function without increasing adverse reactions.
