微通道经皮肾镜碎石取石术治疗结石性脓肾的效果

Microchannel Percutaneous Nephrolithotomy for the Treatment of Calculus

目的分析微通道经皮肾镜碎石取石术(PCNL)治疗结石性脓肾的效果。方法选取2018年1月~12月我院收治的结石性脓肾患者78例,按照随机数字表法分为观察组和对照组,每组39例。对照组行标准通道PCNL治疗,观察组行微通道PCNL治疗,比较两组手术治疗指标(Ⅰ期结石取净率、手术时间、术中出血量、住院时间)和术后并发症总发生率(出血、高热、肾周积液感染、尿培养阳性)。结果两组Ⅰ期结石取净率比较,差异无统计学意义(P>0.05);观察组术中出血量少于对照组,手术时间、住院时间短于对照组,差异有统计学意义(P<0.05)。观察组术后并发症总发生率为7.69%,低于对照组的20.51%,差异有统计学意义(P<0.05)。结论微通道PCNL治疗结石性脓肾疗效显著,能有效减少术中出血量,缩短手术时间、住院时间短,降低术后并发症的发生率,优于标准通道PCNL治疗。

Objective Analysis of the effect of microchannel percutaneous nephrolithotomy(PCNL)in the treatment of calculi.Methods 78 patients with calculous pyonephrosis in our hospital from January to December 2018 were enrolled.The patients were divided into observation group and control group according to random number table,39 cases in each group.The control group underwent standard channel PCNL treatment,and the observation group underwent microchannel PCNL treatment.The surgical treatment indexes(stage I stone removal rate,operation time,intraoperative blood loss,hospitalization time)and total postoperative complications(bleeding,high fever,perirenal effusion infection,positive urine culture)were compared.Results There was no significant difference in the removal rate of the stones stage I of the two groups(P>0.05).The intraoperative blood loss in the observation group was less than that in the control group,the operation time,the hospital stay was shorter than the control group,the difference was statistically significant(P<0.05).The total incidence of postoperative complications in the observation group was 7.69%,which was lower than that in the control group 20.51%,the difference was statistically significant(P<0.05).Conclusion Microchannel PCNL is effective in the treatment of calculous pyonephrosis.It can effectively reduce intraoperative blood loss,shorten the operation time,short hospital stay and reduce the incidence of postoperative complications.It is superior to standard channel PCNL.
