院前急救与急诊科抢救65例急性有机磷农药中毒效果分析

Analysis of the Effect of Prehospital Emergency and Emergency Department in Rescuing 65 Cases of Acute Organophosphorus Pesticide Poisoning

目的分析院前急救与急诊科抢救急性有机磷农药中毒的临床效果。方法 2017年1月-2018年9月期间在该院接受治疗的65例急性有机磷农药中毒患者,按照患者接受治疗方式分为对照组及观察组,对照组27例患者由家属直接送入院到达急诊科后开始接受治疗,观察组38例采取院前抢救及急诊科抢救方式。比较两组临床疗效、呼吸衰竭及阿托品中毒等发生率、机械通气时间、ICU停留时间、CHE活力恢复时间。结果观察组临床治疗总有效率为94.7%,对照组为77.7%,组间差异有统计学意义(χ2=10.06,P<0.05);观察组呼吸衰竭及阿托品中毒发生率低于对照组(P<0.05);观察组机械通气时间、ICU停留时间、CHE活力恢复时间短于对照组(P<0.05)。结论对于急性有机磷农药中毒采取院前急救及急诊科抢救结合方式可提高临床治疗效果,加快患者恢复速度,值得推荐使用。

Objective To analyze the clinical effect of acute organophosphorus pesticide poisoning treated by pre-hospital emergency and emergency department. Methods During January 2017 to September 2018, 65 cases of acute organophosphorus pesticide poisoning patients admitted to our hospital were selected. According to the treatment method, they were divided into the control group and the observation group. The control group(n=27) was sent to the emergency department to start treatment directly by the families, while the observation group(n=38) received pre-hospital rescue and emergency rescue. The clinical efficacy, incidence of respiratory failure and atropine poisoning, mechanical ventilation time, ICU residence time and CHE activity recovery time were compared between the two groups.Results The total effective rate of clinical treatment was 94.7% in the observation group and 77.7% in the control group, and the difference between the two groups was statistically significant(χ2=10.06, P<0.05). The incidence of respiratory failure and atropine poisoning in the observation group was lower than that in the control group(P<0.05). Mechanical ventilation time, ICU residence time and CHE activity recovery time in the observation group were shorter than those in the control group(P<0.05). Conclusion For acute organophosphorus pesticide poisoning, the combination of pre-hospital first aid and emergency department rescue can improve the clinical treatment effect and speed up the recovery of patients, which is worthy of recommendation.
