Frey术式与Whipple术式治疗慢性胰腺炎患者疼痛及生活质量改善的对比研究

Comparative study of Frey’s and Whipple’s operation on pain and quality of life in patients with chronic pancreatitis

目的评估Frey术式与Whipple术式对慢性胰腺炎患者疼痛和生活质量改变的影响。方法选取2011年1月至2012年6月首都医科大学附属北京友谊医院收治的41例慢性胰腺炎患者。31例患者进行了Frey手术,10例患者进行了Whipple手术。回顾性收集两组患者住院期间一般资料、临床资料、术后随访等数据。使用标准化问卷对术后随访期间患者疼痛评分和生活质量进行评估。结果 Whipple组患者手术时间和输注红细胞量高于Frey组。Frey组在疼痛发作频率、疼痛影响工作人数比率、疼痛评分的改善程度优于Whipple组。Frey组在疲劳、恶心和呕吐、缺乏食欲、腹泻症状的改善程度上高于Whipple组。结论对于慢性胰腺炎患者,疼痛和生活质量改善是预后的决定性因素,Frey术后患者可以保持长期无痛状态,并且术后生活质量改善较好。

Objective This study evaluated the effect between Frey's and Whipple's operation on pain and quality of life in chronic pancreatitis. Methods Forty-one patients with chronic pancreatitis undergoing surgical treatment at the Beijing Friendship Hospital were included in our study. 31 patients (75.6%) underwent Frey surgery and 10 patients (24.4%) underwent Whipple surgery. Patient's hospitalization data including general data, clinical data and follow-up data were retrospectively reviewed. The patient's pain score and quality of life were assessed using a standardized questionnaire during follow-up. Results Patients in the Whipple group had higher operation time and red blood cell infusion volume than those in the Frey group. The Frey group showed higher improvement in pain frequency, proportion of pain affecting working population and pain score than Whipple group. The Frey group showed higher improvement in fatigue, nausea and vomiting, lack of appetite, and diarrhea than the Whipple group. Conclusion For patients with chronic pancreatitis, pain and quality of life improvement is a decisive factor in prognosis. Patients who had undergone Fery surgery can maintain long-term painlessness and have better quality of life.
