间接免疫荧光法检测抗核抗体对胃癌患者病情及预后的评价价值

The value of indirect immunofluorescence in the detection of antinuclear antibodies in patients with gastric cancer

目的采用间接免疫荧光法检测抗核抗体(ANA)对胃癌患者病情及预后的评价价值。方法选取本院2012年11月-2015年11月确诊的胃癌患者180作为研究对象,并以同期健康体检者180例作为对照组,采用间接免疫荧光法检测血清中ANA表达类型,探讨血清中ANA表达与患者临床病理特点的关系,并采用COX回归模型探讨ANA表达与胃癌临床预后的关系。结果观察组患者抗核抗体阳性率为26.7%,均质型、斑点型、核仁型表达率分别为6.7%、10.0%、10.0%,明显高于对照组的1.1%、1.1%、1.7%,均质型抗核抗体患者发病年龄越高、分化程度越低、临床分期晚、伴有淋巴结转移的比例更高,TNM分期、淋巴结转移、均质型是胃癌患者预后不良的独立危险因素(P <0.05)。结论胃癌患者抗核抗体表达高,均质型抗核抗体表达的患者胃癌预后差。

Objective To evaluate the value of indirect immunofluorescence ( ANA) in the diagnosis and prognosis of gastric cancer. Methods 180 patients with gastric cancer diagnosed in our hospital from November 2012 to November 2015 were selected as research subjects,and 180 cases of healthy physical examination were used as control group. The expression of ANA in serum was detected by indirect immunofluorescence,and the relationship between the expression of ANA in the serum tissue and the characteristics of patients with the bed were discussed,and the COX regression model was used to investigate the relationship between the expression of ANA and the clinical prognosis of gastric cancer. Results The positive rate of anti nuclear antibody in the observation group was 26. 7%,the expression of homogenous type,speckle type and nucleolar type ( 6. 7%,10. 0% and 10. 0%) were respectively higher than that in the control group ( 1. 1%,1. 1% and 1. 7%). The higher the age of the patients,the lower the degree of differentiation,the late clinical stage,and the higher proportion of the lymph node metastasis,TNM score. Stage,lymph node metastasis,and homogeneity were independent risk factors for poor prognosis in patients with gastric cancer ( P < 0. 05). Conclusion Gastric cancer patients with high expression of antinuclear antibodies and poor expression of homogeneous antinuclear antibodies have poor prognosis.
