合并糖尿病股骨颈骨折患者的手术治疗效果

The Effect of Surgical Treatment of Patients With Diabetic Femoral Neck Fracture

目的探讨合并糖尿病股骨颈骨折患者的外科手术治疗及效果。方法纳入2015年1月-2018年12月的100例合并糖尿病股骨颈骨折患者,随机分组。空心钉内固定治疗组选择空心钉内固定手术,髋关节置换手术治疗组选择髋关节置换手术。分析合并糖尿病股骨颈骨折患者的外科手术治疗时间、手术平均出血量、手术后开始活动的时间;治疗前后患者髋关节功能指标、自理ADL评分;并发症。结果髋关节置换手术治疗组髋关节功能指标、自理ADL评分、手术后开始活动的时间、并发症和空心钉内固定治疗组比较有优势,P<0.05。髋关节置换手术治疗组的外科手术治疗时间、手术平均出血量均高于空心钉内固定治疗组,P<0.05。结论合并糖尿病股骨颈骨折患者实施髋关节置换手术出血较多,且手术时间较长,但可获得较好效果。

Objective To investigate the surgical treatment and effect of patients with diabetic femoral neck fracture.Methods 100 patients with femoral neck fractures with diabetes mellitus from January 2015 to December 2018 were randomized.Hollow nail internal fixation treatment group was treated with cannulated screw internal fixation,and hip replacement surgery treatment group was selected for hip replacement surgery.The time of surgical treatment,the average amount of bleeding in the operation,and the time after the operation of the patients with diabetic femoral neck fracture were analyzed.The hip joint function index,the concise health scale score,the self-care ADL score before and after treatment,and complications were analyzed.Results The hip function index,the concise health scale score,the self-care ADL score,the time of onset of surgery,the complications,and the cannulated screw fixation group were superior in the hip replacement surgery group(P<0.05).The surgical treatment time and average blood loss of the hip replacement surgery group were higher than those of the open screw fixation group(P<0.05).Conclusion Patients with diabetic femoral neck fractures have more bleeding under hip replacement surgery,and the operation time is longer,but good results can be obtained.
