喉返神经监测在甲状腺癌再次手术中的应用

Application of Intraoperative Recurrent Laryngeal Nerve Monitoring in Reoperation of Thyroid Cancer

目的分析喉返神经监测在甲状腺癌再次手术中的应用效果。方法选取2017年8月-2019年2月我院收治的甲状腺癌再次复发手术患者77例为研究对象,随机摸球法分为对照组38例为常规手术,观察组39例在常规手术基础上进行术中喉返神经监测,对比两种治疗方式的效果。结果观察组患者喉返神经损伤和甲状旁腺损伤均少于对照组,差异有统计学意义(P<0.05);观察组患者临床并发症率少于对照组,差异有统计学意义(P<0.05);观察组患者术后住院时间和引流拔管时间等均更短,差异有统计学意义(P<0.05)。结论为甲状腺癌再次手术患者手术时进行术中喉返神经监测可有效降低喉返神经损伤,控制术后并发症。

Objective To analyze the effect of recurrent laryngeal nerve monitoring in thyroid cancer reoperation.Methods 77 patients with recurrence of thyroid cancer treated in our hospital from August 2017 to February 2019 were selected as the research object.38 patients in the control group were treated with routine operation by random touching ball method.39 patients in the observation group were monitored with intraoperative recurrent laryngeal nerve on the basis of routine operation,and the effects of the two treatment methods were compared.Results The recurrent laryngeal nerve injury and parathyroid injury in the observation group were less than those in the control group,the difference was statistically significant(P<0.05);the clinical complication rate in the observation group was significantly less than that in the control group,the difference was statistically significant(P<0.05);the hospitalization time and drainage and extubation time in the observation group were shorter,the difference was statistically significant(P<0.05).Conclusion Intraoperative monitoring of recurrent laryngeal nerve can effectively reduce the incidence of recurrent laryngeal nerve injury and control the complications in patients undergoing reoperation for thyroid cancer.
