盆腔表浅型子宫内膜异位症患者应用不同降调节方案IVF/ICSI-ET结局的回顾性分析

Outcomes of peritoneal endometriosis patients with different IVF/ICSI pituitary down regulation protocols:a retrospective study

目的探讨盆腔表浅型子宫内膜异位症患者行不同降调节方案的IVF/ICSI-ET结局。方法根据纳入和排除标准,收集2011~2014年在中山大学孙逸仙纪念医院生殖中心行IVF/ICSI-ET助孕的盆腔表浅型子宫内膜异位症患者为研究对象,共纳入238个周期。根据降调节方案不同分成两组:超长方案组(n=45)和长方案组(n=193),比较两组的促排卵结局和妊娠结局。结果超长方案组和长方案组的获卵数[(12.62±6.79)vs.(12.18±7.05)]、可利用胚胎数[(7.17±4.53)vs.(7.64±5.29)]、移植胚胎数[(2.15±0.56)vs.(2.16±0.50)]、种植率(42.47%vs.32.47%)和临床妊娠率(55.88%vs.52.80%)比较均无显著性差异(P>0.05)。两组患者的早期流产率和卵巢过度刺激综合征发生率比较亦无显著性差异(P>0.05)。结论对于盆腔表浅型子宫内膜异位症患者,应用长方案降调节可获得与超长方案相似的妊娠结局。

Objective:To investigate the impact of different pituitary down-regulation protocols on the IVF/ICSI-ET outcomes of peritoneal with peritoneal endometriosis.Methods:The data of the patients with peritoneal endometriosis treated with IVF/ICSI-ET in the Reproductive Center of Sun Yat-sen Memorial Hospital from January 2011 to December 2014 were retrospectively analyzed.A total of 238 cycles were included and which were divided into two groups:ultra-long down-regulation protocol group(n=45)and the long down-regulation protocol group(n=193).Results:There were no significantly differences in the number of oocytes retrieved[(12.62±6.79)vs.(12.18±7.05)],the number of available embryos[(7.17±4.53)vs.(7.64±5.29)],the number of transplanted embryos[(2.15±0.56)vs.(2.16±0.50)],implantation rate(42.47%vs.32.47%)and clinical pregnancy rate(55.88%vs.52.80%)between the ultra-long protocol group and long protocol group(P>0.05).There was also no significant difference in the early abortion rate and ovarian hyperstimulation syndrome rate between the two groups(P>0.05).Conclusions:For patients with peritoneal endometriosis,ultra-long pituitary down regulation protocol can achieve pregnancy outcomes similar to those of long pituitary down-regulation protocol.
