心型脂肪酸结合蛋白与心肌酶谱在轮状病毒感染患儿心肌损伤诊断的价值比较

Comparison of heart-type fatty acid binding protein and myocardial enzymes in diagnosis of myocardial injury in rotavirus children

目的探讨心型脂肪酸结合蛋白(H-FABP)与心肌酶谱诊断轮状病毒感染患儿心肌损伤的价值。方法选取该院轮状病毒感染患儿80例,于入组3~<6h、6~12h检测肌酸激酶(CK)、CK同工酶(CK-MB)、血清肌钙蛋白(cTnI)、天门冬氨酸氨基转移酶(AST)、血清乳酸脱氢酶(LDH)及H-FABP水平,比较心肌损伤与心肌正常患儿H-FABP、CK、CK-MB、cTnI、AST及LDH的水平及阳性率。结果3~<6h、6~12h轮状病毒感染心肌损伤患儿的H-FABP、CK、CK-MB、cTnI、AST及LDH水平显著高于心肌正常组患儿(P<0.05);轮状病毒感染心肌损伤患儿入组3~<6h、6~12h的H-FABP、CK-MB、cTnI阳性率显著高于心肌正常患儿(P<0.05);H-FABP在诊断轮状病毒感染患儿入组3~<6h、6~12h心肌损伤的约登指数均为最高,分别为83.02%、78.00%。结论H-FABP对轮状病毒感染患儿心肌损伤的诊断价值高于心肌酶谱。

Objective To explore the value of heart-type fatty acid binding protein(H-FABP)and myocardial zymogram in diagnosis of myocardial injury in children with rotavirus.Methods A total of 80 children with rotavirus infection in this hospital were selected.The levels of creatine kinase(CK),CK-MB,serum troponin(cTnI),aspartate aminotransferase(AST),lactate dehydrogenase(LDH)and H-FABP were measured at 3-<6 and 6-12 h after admission.The levels and positive rates of H-FABP,CK,CK-MB,cTnI,AST and LDH in patients with myocardial injury and normal myocardium were compared.Results The levels of H-FABP,CK,CK-MB,cTnI,AST and LDH in children with myocardial injury caused by rotavirus infection at 3-<6 h and 6-12 h were significantly higher than those in children with normal myocardium(P<0.05).The positive rates of H-FABP,CK-MB,and cTnI in children with myocardial injury caused by rotavirus infection for 3-<6 h and 6-12 h were significantly higher than those in children with normal myocardium(P<0.05).The Hadin index of H-FABP in diagnosing myocardial injury in children with rotavirus at 3-<6 h and 6-12 h was the highest,which was 83.02%and 78.00%respectively.Conclusion The diagnostic value of H-FABP for myocardial injury in children with rotavirus is higher than that of myocardial zymogram.
