摘要 总结芦连菊教授治疗放射性肺炎的学术思想和临证经验,认为放射性肺炎发生与正虚、热毒、痰瘀、津伤密切相关,放射线作为特殊火热毒邪耗气伤津,易损伤肺络,基本病机为肺脾气虚、热毒炽盛、痰热瘀阻、气阴两虚。在审证求因、治病求本的基础上,其治疗围绕"气、血、津"的生成和运行特点,以"调气、调血、调津"为基础,针对其复杂病证变化分段论治,灵活运用加味救肺饮、解毒清营汤、清金化痰汤、沙参麦冬汤加减化裁,力求恢复肺的正常生理功能,同时兼顾脏腑之间的协调性。 Summarizing Professor LU Lian-ju’s academic thoughts and clinical experience on radiation pneumonitis.Professor LU Lian-ju considers the occurrence of radiation pneumonitis is related to weakness of the body,heat-toxicity,phlegm stagnation and deficiency of body fluid closely.Radiation,as a special kind of heat-toxicity,causes the deficiency of Qi and body fluid.It also damages lung collaterals,and both of the two results above lead to the disease.The basic pathogenesis of the disease are lung-spleen deficiency,toxic heat flourishing,phlegm-heat stasis and deficiency of Qi and Yin.In treatment,based on the principles of determination of etiologic factors based on differentiation and searching for the primary cause of disease,considering the characteristics of the generation and movement of Qi,blood and body fluid,taking the basic measures as regulating Qi,blood and body fluid.Then taking different measures based different stages of the disease and complex syndromes such as "Modified Jiufei Decoction","Jiedu and Qingying Decoction","Qingjin and Huatan Decoction" and "Shashen and Maidong Decoction", then introducing the decoctions above flexibly in order to recover the normal physiological function of lung and coordinate visceral functions.
