IL-22在原发性干燥综合征患者唇腺中的表达及意义 被引量:3

The expression and clinical significance of interleukin-22 in labial salivary glands of patients with pri-mary Sjogren's syndrome

摘要 目的探讨白细胞介素(interleukin,IL)-22在原发性干燥综合征(primary Sj?gren's syn-drome,pSS)患者唇腺组织中的表达和意义,以及其与唇腺组织中淋巴细胞浸润和C反应蛋白(C-reactive protein,CRP)、免疫球蛋白(immunoglobulin,Ig)、红细胞沉降率(erythrocyte sedimentation rate,ESR)、抗SSA抗体(anti-SSA antibody,SSA)、抗SSB抗体(anti-SSB antibody,SSB)的相关性.方法选取2016年8月至2017年12月在滨州市人民医院风湿免疫科诊治的30例pSS患者作为观察组,同时随机选择10例在医院口腔科门诊就诊的颌面部外伤或唇腺囊肿的患者作为对照组.采用免疫组化法对两组研究对象的唇腺组织进行IL-22染色,采用HE染色法检测唇腺组织中淋巴细胞浸润,分析IL-22表达与唇腺组织中淋巴细胞浸润情况,并分析IL-22与ESR、CRP、Ig、抗SSA抗体、抗SSB抗体之间的相关性.结果 IL-22在pSS患者唇腺组织中高表达,主要表达于导管上皮细胞、腺泡上皮细胞和淋巴细胞浸润区.pSS组IL-22阳性表达数为28例,阳性表达率为93%,对照组阳性表达数为1例,阳性表达率为10%.两组IL-22阳性细胞表达数比较,差异具有统计学意义(t=8.73,P<0.05).IL-22阳性细胞数与淋巴细胞浸润程度呈正相关(r=0.853,P<0.05),IL-22在自身抗体(SSA及SSB)阳性的唇腺组织中的阳性细胞数明显高于自身抗体阴性组,差异具有统计学意义(t=-7.64,P<0.05),pSS组中IL-22阳性细胞数与CRP、IgG、IgM、IgA、ESR无相关性(P>0.05).结论 IL-22在pSS患者唇腺组织中呈高表达,可能在pSS的发病过程中起重要作用. Objective To investigate the expression of interleukin(IL)-22 in the labial salivary glands of patients with primary Sj?gren's syndrome(pSS)and its relationship with the infiltration of lymphocytes among the gland,C-reactive protein(CRP),immunoglobulin(Ig),erythrocyte sedimentation rate(ESR),anti SSA anti-body and anti-SSB antibodies. Methods Thirty patients with pSS were selected from the department of rheuma-tology and immunology of Binzhou People's Hospital from August 2016 to December 2017 as observation group, and 10 patients with maxillofacial trauma or lip gland cyst were randomly selected as control group. The labial gland tissues of the two group subjects were stained with IL-22 by immunohistochemical method. The lymphocyt-ic infiltration in labial gland was detected by HE staining. The correlation between IL-22 and lymphocytic infil-tration in labial gland were analyzed,and then the correlation between IL-22 and ESR,CRP,Ig,anti-SSA anti-body and anti-SSB antibody was analyzed. Results The higher expression of IL-22 was found in the labial sali-vary glands of pSS patients,mainly in ductal epithelial cells,acinar epithelial cells and lymphocytic infiltrating areas. The positive expression rate of IL-22 in pSS group was 93 % with 28 cases. The positive expression rate of IL-22 in the control group was 10 % with 1 case. The significant difference has been found in expression of IL-22 positive cells between the two groups(t = 8. 73,P < 0. 05). The number of IL-22 positive cells was positively correlated with lymphocyte infiltration(r = 0. 853,P < 0. 05). The number of IL-22 positive cells in autoantibody positive labial gland tissue was significantly higher than that in autoantibody negative group (t =- 7. 64,P < 0. 05). The number of IL-22 positive cells in pSS group was not correlated with CRP,IgG,IgM,IgA and ESR (P > 0. 05). Conclusion IL-22 is highly expressed in the labial salivary glands of pSS patients, which may play an important role in the pathogenesis of pSS.
