一次性宫腔压迫球囊填塞术治疗产后出血的效果分析

Effect analysis of one-time intrauterine compression balloon tamponade for postpartum hemorrhage

目的:探究用一次性宫腔压迫球囊填塞术治疗产后出血的效果.方法:将2017年3月至2018年3月在绵阳市人民医院接受治疗的100例产后出血患者作为研究对象.采用随机双盲法将这些患者分为纱布填塞组和球囊填塞组(50例/组).为纱布填塞组患者采用子宫腔纱布填塞术进行治疗,为球囊填塞组患者采用一次性宫腔压迫球囊填塞术进行治疗.然后比较两组患者止血的效果、手术的时间、术中的出血量、术后24 h其血红蛋白的水平及其接受子宫动脉结扎和子宫切除的情况.结果:1)治疗后,球囊填塞组患者止血的成功率为98%(49/50),纱布填塞组患者止血的成功率为82%(41/50),组间差异有统计学意义(P<0.05).2)与纱布填塞组患者相比,球囊填塞组患者术中的出血量较少,术后24 h其血红蛋白的水平较高,其手术的时间较短,差异有统计学意义(P<0.05).3)球囊填塞组患者中接受子宫动脉结扎患者的占比、接受子宫切除患者的占比均低于纱布填塞组患者,差异有统计学意义(P<0.05).结论:用一次性宫腔压迫球囊填塞术治疗产后出血的效果较好,可快速地减少患者子宫的出血量,改善其预后.

objective: to explore the effect of one-time intrauterine compression balloon tamponade on postpartum hemorrhage. Methods: 100 patients with postpartum hemorrhage who were treated in mianyang people’ s hospital from March 2017 to March 2018 were selected as research objects. The patients were divided into gauze packing group and balloon packing group (50 cases/group) by randomized double-blind method. Patients in the gauze packing group were treated with uterine cavity gauze packing, and patients in the balloon packing group were treated with one-time uterine cavity compression balloon packing. Then, the hemostatic effect, time of operation, intraoperative blood loss, hemoglobin level 24 h after surgery, and hysterectomy and uterine artery ligation were compared between the two groups. Results: 1) after treatment, the success rate of hemostasis was 98%(49/50) in the balloon packing group and 82%(41/50) in the gauze packing group, with statistically significant differences between the groups (P<0.05). 2) compared with patients in the gauze packing group, patients in the balloon packing group had less intraoperative blood loss, higher hemoglobin level 24 h after surgery, and shorter operative time, with statistically significant differences (P < 0.05). 3) the proportion of patients receiving uterine artery ligation and hysterectomy in the balloon packing group was lower than that in the gauze packing group, with statistically significant differences (P<0.05). Conclusion: one-time intrauterine compression balloon tamponade is effective in the treatment of postpartum hemorrhage.
