摘要
目的调查肺癌患者治疗相关性症状,并分析其影响因素,为症状管理提供参考。方法采用一般资料调查表、Karnofsky评分量表、治疗相关性症状列表(TRSC)及简易应对方式量表(BCI)对302例肺癌患者进行调查。结果肺癌患者TRSC总分为(20.62±9.10)分,其中放疗患者为(14.73±8.13)分,化疗患者为(19.12±7.95)分,放化疗患者为(23.45±9.02)分。回归分析结果显示,治疗方案、肿瘤TNM分期和拒绝、计划、积极应对及使用有用的支持应对方式是肺癌患者治疗相关性症状的主要影响因素,共解释TRSC总分变化的33.70%。结论肺癌患者承担较严重的治疗相关性症状困扰,联合放化疗及化疗患者、Ⅳ期患者及采用拒绝应对方式者症状更严重。医护人员应指导患者采用积极应对方式以缓解治疗相关性症状。
Objective To investigate therapy-related symptoms among lung cancer patients,and analyze its influencing factors,so as to provide reference for symptom management.Methods Totally 302 lung cancer patients were investigated using Karnofsky performance score,Therapy-related Symptom Checklist(TRSC)and Brief COPE Inventory(BCI).Results The total TRSC score among lung cancer patients was(20.62±9.10),which was(14.73±8.13)for the patients undergoing radiotherapy,(19.12±7.95)for the patients suffering chemotherapy,and(23.45±9.02)for the other patients experiencing combined chemoradiotherapy.The result of regression analysis indicated that,treatment regimen,cancer TNM stage,and the coping style of refusal,planning,positive coping and using useful support were main influencing factors of therapy-related symptoms among lung cancer patients,which totally explained 33.70%variance of TRSC score.Conclusion Lung cancer patients suffer serious therapy-related symptom distress,and the patients undergoing combined chemoradiotherapy or only chemotherapy,with advanced stageⅣcancer and using refusal coping style are prone to encounter more serious symptoms.Medical staff should guide cancer patients to apply positive coping styles to relieve therapy-related symptoms.
