经鼻同步间歇指令通气在重度呼吸窘迫综合征早产儿撤机后的应用

摘要 目的:探讨重度呼吸窘迫综合征(RDS)早产儿撤机后应用经鼻同步间歇指令通气的效果。方法:选择本院新生儿重症监护病房2018年1月至2019年1月收治的重度RDS早产儿186例,以数字表为参考将早产儿划分为研究组与对照组,每组各93例。对照组早产儿撤机后接受经鼻持续气道正压通气,研究组早产儿撤机后被施以经鼻同步间歇指令通气,对比两组早产儿治疗后的血气分析指标。结果:研究组早产儿的氧分压(PaO2)、二氧化碳分压(PaCO2)、氧合指数(OI)、pH值以及血氧饱和度(SaO2)与对照组早产儿进行比较,数据优势显著,组间差异存在统计学意义(t值分别为4.100、4.653、3.086、15.333、9.432,P<0.05)。结论:重度呼吸窘迫综合征早产儿撤机后施以经鼻同步间歇指令通气,可以改善早产儿的血气分析指标,优化早产儿的肺通气功能。 Objective:To investigate the effect of nasal synchronized intermittent mandatory ventilation after retraction of severe respiratory distress syndrome (RDS) in preterm infants. Methods: A total of 186 preterm infants with severe RDS who were admitted to the neonatal intensive care unit from January 2018 to January 2019 were enrolled. The premature infants were divided into study group and control group, with 93 cases in each group. In the control group, the premature infants underwent nasal continuous positive airway pressure after weaning. The premature infants in the study group were given nasal synchronized intermittent ventilation after the machine was withdrawn. The blood gas analysis indexes of the two groups of preterm infants were compared. Results: The oxygen partial pressure (PaO 2), carbon dioxide partial pressure (PaCO 2), oxygenation index (OI), pH and blood oxygen saturation (SaO 2) of the preterm infants in the study group were compared with the preterm infants in the control group. The data superiority was significant.The statistical significance of the difference was very significant ( t values were 4.100, 4.653, 3.086, 15.333, 9.432, P < 0.05). Conclusion: Severe respiratory distress syndrome premature infants with nasal intermittent intermittent mandatory ventilation, can improve the blood gas analysis indicators of premature infants, optimize lung ventilation function in premature infants, it is worthy of clinical application.
