基于磁共振弥散张量成像在脊髓型颈椎病中的应用价值研究 被引量:1

摘要 目的探讨磁共振(MRI)弥散张量成像(DTI)在脊髓型颈椎病中的应用价值。方法将我院2015年1月~2018年12月收治的82例脊髓型颈椎病(CSM)患者根据CSM病情轻重程度分为轻度组36例,重度组46例,选取同期45例健康体检者设为对照组。对3组观察对象的颈部不同部位行常规MRI及DTI成像;分析3组观察对象颈部各部位表观扩散系数(ADC)、颈髓不同层面向异性(FA)。结果重度组ADC值明显高于轻度组与对照组,且轻度组ADC值明显高于对照组(P<0.05);重度组FA值明显低于轻度组与对照组,且轻度组FA值明显低于对照组(P<0.05)。结论 DTI能够更好的鉴别健康人群、轻度CSM及重度CSM的ADC与FA值,对CSM的早期诊断及病情评估中具有重要价值。 Objective To investigate the application value of diffusion tensor imaging (DTI) in cervical spondylotic myelopathy(CSM).Methods 82 patients with cervical spondylotic myelopathy (CSM) admitted to our hospital from January 2015 to December 2018 were divided into mild group (36 cases) and severe group (46 cases) according to the severity of CSM.Routine MRI and DTI were performed on different parts of the neck of three groups of subjects. Apparent diffusion coefficient(ADC) and anisotropy of different layers of the cervical spinal cord(FA) were analyzed in three groups.Results The ADC value of severe group was significantly higher than that of mild group and control group, and the ADC value of mild group was significantly higher than that of control group(P<0.05);the FA value of severe group was significantly lower than that of mild group and control group, and the FA value of mild group was significantly lower than that of control group(P<0.05).Conclusion DTI can better differentiate ADC and FA values of healthy people,mild CSM and severe CSM,and has important value in early diagnosis and disease evaluation of CSM.
