EB病毒抗体联合应用CT及MRI对鼻咽癌诊断价值 被引量:3

摘要 目的 探讨脱氧核糖核酸病毒(EB病毒)抗体联合应用电子计算机断层扫描(CT)及磁共振成像(MRI)对鼻咽癌诊断价值。方法 选择我院2015年4月~2017年4月150例鼻咽癌患者进行分析,分为CT组、MRI组、Eb病毒组、CT+Eb病毒组、MRI+Eb病毒组,每组各30例,检测患者血清EB病毒衣壳抗原抗体(VAC-IgA)、早期抗原抗体(EA-IgA),采用螺旋CT及MRI检查患者鼻咽部,以病理检查结果为金标准,将所有检查结果进行比较,对EB病毒抗体、CT及MRI的阳性检出率、灵敏度、特异性进行分析。结果 鼻咽癌患者中MRI+Eb病毒组检出30例(100.0%),CT+Eb病毒组检出27例(90.0%)、CT组检出25例(83.3%)、MRI组检出26例(86.7%),Eb病毒组检出26例(86.7%)。CT组灵敏度83.3%,特异性86.7%;MRI组灵敏度86.7%,特异性90.0%;Eb病毒组的灵敏度90.0%,特异性93.3%;CT+Eb病毒组的灵敏度93.3%,特异性96.7%;MRI+Eb病毒组的灵敏度93.3%,特异性96.7%。结论 EB病毒抗体、CT及MRI是检查鼻咽癌的有效方法,具有较高的阳性检出率、灵敏度、特异性,将EB病毒抗体联合应用MRI对鼻咽癌早期筛查具有更好的检查效果。 Objective To study the diagnostic value of EB virus antibody combined with computed tomography (CT) and magnetic resonance imaging (MRI) in nasopharyngeal carcinoma (NPC). Methods From April 2015 to April 2017, 150 patients with NPC in our hospital were studied. They were divided into CT group, MRI group, Eb virus group, CT+Eb virus group and MRI+Eb virus group (n=30). Serum EB, capsid antigen (VAC-IgA) and early antigen antibody (EA-IgA) were detected. Multidetector row CT and magnetic resonance imaging (MRI ) were used to examine the nasopharynx of the patient. The results of pathological examination were the gold standard, and all the results were compared. The positive rate, sensitivity and specificity of antibodies against EB virus, CT and MRI were analyzed. Results MRI +Eb virus in patients with nasopharyngeal carcinoma group was 30 cases (100%), CT+Eb virus group was 27 cases (90%), CT group was 25 cases (83.3%), MRI group was 26 cases (86.7%), Eb virus group was 26 cases (86.7%) in group. CT group with sensitivity was 83.3%, specificity was 86.%;MRI group with sensitivity was 86.7%, specificity was 90%;Eb virus group with sensitivity was 90%, specificity was 93.3%;CT+Eb virus group with sensitivity was 93.3%, specificity was 96.7%;the sensitivity and specificity of the MRI +Eb toxic group was 93.3% and 96.7%, respectively. Conclusion EB virus antibodies, CT and MRI were effective methods for the detection of nasopharyngeal carcinoma, with high positive detection rate, sensitivity and specificity. The combination of EB antibody and CT and MRI had better effect on early screening of nasopharyngeal carcinoma.
