低剂量螺旋CT联合肺癌自身抗体对早期肺癌的诊断价值 被引量:10

摘要 目的 探讨低剂量螺旋CT联合P53、PGP9.5、SOX2、GAGE7、GBU4-5、MAGEA1和CAGE7项自身抗体在早期肺癌中的诊断价值。方法 收集山东省立医院2017年7月~2019年3月就诊的肺小结节患者144例,抽血化验肺癌自身抗体,且均经病理学检查确诊。病理诊断为肺癌的患者(肺癌组)85例,病理诊断为良性病变(良性组)59例。收集同期于山东省立医院健康查体中心查体的74例查体者入选为健康(对照组),均行低剂量螺旋CT检查,并且未发现肺内结节,且均抽血化验肺癌自身抗体。对比低剂量螺旋CT联合肺癌自身抗体对早期肺癌的诊断率。结果 低剂量螺旋CT对肺癌的诊断阳性率(89.4%),但也存在较高的假阳性率(35.6%)。低剂量螺旋CT联合检测肺癌自身抗体后,诊断肺癌的阳性率可进一步提高(92.9%),且假阳性率明显降低(18.6%)。结论 低剂量螺旋CT联合肺癌自身抗体检测对早期肺癌有较高的临床诊断价值。 Objective To investigate the diagnostic value of low-dose spiral CT combined with P53, PGP9.5, SOX2, GAGE7, GBU4-5, MAGE A1 and CAGE autoantibodies in early lung cancer. Methods 144 patients with pulmonary nodules were collected from Shandong Provincial Hospital from July 2017 to March 2019. Blood samples were taken to test autoantibodies of lung cancer, and all of them were confirmed by pathological examination. Pathological diagnosis of lung cancer (lung cancer group) was 85 cases, pathological diagnosis of benign lesions (benign group) was a total of 59 cases. Seventy-four healthy subjects (control group) were enrolled in the same period from the health examination center of Shandong Provincial Hospital. All patients underwent low-dose spiral CT examination and no pulmonary nodules were found, and all of them were taken blood to test lung cancer autoantibodies. We compared the diagnostic rate of low-dose spiral CT combined with lung cancer autoantibody for early lung cancer. Results The positive rate of low dose spiral CT in the diagnosis of lung cancer was high (89.4%), but there was also a high false positive rate (35.6%). The positive rate of diagnosing lung cancer can be further increased (92.9%) and the false positive rate can be significantly reduced (18.6%) after low-dose spiral CT combined with autoantibody detection. Conclusion Low-dose spiral CT combined with lung cancer autoantibody detection has high clinical diagnostic value for early lung cancer.
