探讨非支气管动脉性体动脉分支在咯血栓塞治疗中的价值

Value of non-bronchial systemic artery in hemoptysis of embolization therapy

目的 探讨非支气管动脉性体动脉分支(NBSA)作为责任血管参与咯血的重要性,为咯血栓塞治疗提供指导。方法 回顾分析我院2014年12月~2017年5月84例咯血患者的临床资料及多排螺旋CT血管成像(CTA)、DSA、随访等资料,了解非支气管性体动脉作为责任血管在咯血栓塞治疗中的重要性。结果 84例患者中,共发现责任血管236支,其中NBSA64支(64/236,27%),且均参与咯血,来源于肋间动脉27支,胸廓内动脉17支,胸外侧动脉3支,膈动脉11支,甲状颈干2支,食道动脉4支;其中有5例患者因漏栓NBSA短期内复发。结论 NBSA是重要的咯血相关动脉,漏栓NBSA或栓塞不彻底是咯血短期复发的重要原因之一。

Objective To discribe the importance of non-bronchial systemic artery (NBSA) as the responsible vessels with hemoptysis,and to guide interventional embolization therapy for hemoptysis. Methods Clinical, multi-slice CT angiography, digital subtract angiography (DSA) and follow-up data of 84 patients with hemoptysis at our hospital from December 2014 to May 2017 were retrospectively analyzed, so as to realize the importance of NBSA as the responsible vessels in hemoptysis embolization therapy. Results A total of 236 responsible vessels were found of 84 patients, of which 64 were NBSA(27%), and all were involved in hemoptysis. They arised from intercostal arteries (27), internal thoracic arteries (17), arteriae thoracalis lateralis (3), phrenic artery (11), thyroid axis (2), and esophageal artery (4);5 patients were recurrent because of omission of NBSA in a short time. Conclusion NBSA are the important artery with hemoptysis, omission of NBSA or not complete embolism is one of the important reasons for recurrent hemoptysis in a short time.
