电刺激联合穴位注射对重症脑炎持续昏迷患儿的影响

Effect of electric stimulation combined with acupoint injection on the recovery of nerve function and quality of life in children with severe encephalitis and persistent coma

目的探讨电刺激联合穴位注射对重症脑炎持续昏迷患儿神经功能恢复及生活质量的影响.方法选取漯河市中心医院2014-04—2016-02重症脑炎持续昏迷患儿63例,依据建档顺序分组,对照组31例予以电刺激治疗,研究组32例予以电刺激与穴位注射联合治疗,2组均持续治疗20 d,对比2组治疗前后神经功能指标[血清神经生长因子(N G F)、髓磷脂碱性蛋白(MBP)、神经元特异性烯醇化酶(NSE)、S100B蛋白]水平及生活质量评分情况,以及治疗前后昏迷指数(GCS)、神经功能缺损程度(NIHSS)评分及治疗后清醒率.结果研究组治疗后血清NGF、MBP、NSE、S100B蛋白水平低于对照组,差异有统计学意义(P<0.05);研究组治疗后GCS评分高于对照组,NIHSS评分低于对照组,差异有统计学意义(P<0.05);研究组治疗后清醒率84.38%(27/32)高于对照组61.29%(19/31),差异有统计学意义(P<0.05);研究组治疗后生理功能、认知功能、躯体功能、情感职能、精神健康、总体健康等生活质量评分高于对照组,差异有统计学意义(P<0.05).结论电刺激联合穴位注射治疗重症脑炎持续昏迷患儿,可显著改善其神经功能,提高清醒率,提升生活质量.

Objective To investigate the effect of electric stimulation combined with acupoint injection on the recovery of nerve function and quality of life in children with severe encephalitis and persistent coma. Methods A total of 63 cases of severe encephalitis and persistent coma in our hospital from April 2014 to February 2016 were selected and grouped according to the filing sequence,31 cases in the control group treated by electric stimulation,and 32 cases in the study group treated by electric stimulation and acupoint injection.The 2 groups were treated continuously for 20 d.The nerve function indexes[serum nerve growth factor (NGF) and myelin basic protein (MBP),neuron specific enolase (NSE) and S100B protein]and quality of life score of 2 groups before and after treatment were analyzed.The coma index (GCS),the degree of nerve function defect (NIHSS) before and after treatment and the wakefulness rate after treatment were compared between the 2 groups. Results The serum levels of NGF,MBP,NSE and S100B in the study group were lower than those in the control group,and the difference was statistically significant( P <0.05).After treatment,the GCS score of the study group was higher than that of the control group,and the NIHSS score was lower than that of the control group.The difference was statistically significant( P <0.05).The wakefulness rate of the study group was 84.38%(27/32) higher than that of the control group[61.29%(19/31)],and the difference was statistically significant( P <0.05).After treatment,the scores of life quality of physiological function,cognitive function,physical function,emotional function,mental health,general health and other aspects of the study group were higher than those of the control group,and the difference was statistically significant( P <0.05). Conclusion Electrical stimulation combined with acupoint injection in children with severe encephalitis and persistent coma can significantly improve the neurological function,improve the wakefulness rate and improve the quality of life.
