综合保温护理预防胃癌术中低体温效果观察

Observation on the effect of comprehensive thermal insulation care on prevention of hypothermia in gastric cancer

目的探讨综合保温护理措施预防胃癌术中低体温的效果。方法前瞻性选取2017年1月~2018年12月于本院行胃癌根治术的患者60例,按照随机配对原则分为常规保温组和复合保温组,各30例,常规保温组采取棉垫方式保温,复合保温组通过患者身下放置电热毯,加热手术用液等方式维持体温。比较两组患者不同时间点的直肠温度、低体温发生率、麻醉后复苏时间、术后恢复情况等。结果复合保温组患者麻醉后各时间点直肠温度均高于常规保温组(P<0.05),术中低体温率及术后感染率略低于常规保温组,但差异无统计学意义(P>0.05),复合保温组术后复苏时间、拔管时间和住院时间明显短于常规保温组,差异有统计学意义(P<0.05)。结论综合保温护理有助于行胃癌根治术过程中患者体温的维持,降低低体温率,降低术后感染风险,能够加快麻醉苏醒时间,加快术后恢复进程,值得在临床中推广应用。

Objective To investigate the effect of comprehensive thermal insulation care on prevention of hypothermia in gastric cancer. Methods Sixty patients with radical gastrectomy in our hospital from January 2017 to December 2018 were prospectively selected. According to the random matching principle, they were divided into conventional thermal insulation group(n=30) and composite thermal insulation group(n=30). The conventional thermal insulation group adopted cotton pad insulation. The composite thermal insulation group maintained the body temperature by placing an electric blanket under the patient's body and heating the surgical fluid. The rectal temperature, hypothermia incidence, recovery time after anesthesia, and postoperative recovery between the two groups at different time points was compared. Results The rectal temperature of the patients in the composite thermal insulation group was higher than that in the conventional thermal insulation group(P<0.05). The intraoperative low body temperature rate and postoperative infection rate in the composite thermal insulation group were slightly lower than that of the conventional thermal insulation group, but the differences were not statistically significang(P>0.05). The postoperative recovery time, extubation time and hospital stay in the composite thermal insulation group was significantly shorter than that of the conventional thermal insulation group, and the differences were statistically significant(P<0.05). Conclusion Comprehensive thermal insulation care can help maintain the patient's body temperature during radical gastrectomy, reduce the hypothermia rate, reduce the risk of postoperative infection, accelerate the recovery time of anesthesia, and speed up the postoperative recovery process. It is worthy of popularization in clinical practice.
