影响CAD/CAM高嵌体试戴效果的回顾性分析

摘要 目的对CAD/CAM系统制作的下颌第一磨牙高嵌体试戴效果进行回顾性分析,旨在为高嵌体邻面设计及远期临床效果提供依据和指导。方法回顾我院2017年1月-2019年1月完成下颌第一磨牙根管治疗需做高嵌体92名患者(92颗高嵌体),其中分为试戴时就位顺利组(46颗高嵌体)以及就位困难组(需磨除部分邻面瓷体才可就位,46颗高嵌体),比较在CAD/CAM系统生成的修复体二维图像中邻面接触区黄色区域与绿色区域的面积比,分析影响高嵌体试戴效果的相关因素。结果修复体邻面接触区黄色区域与绿色区域面积比值在在近中面为:0.0378±0.0261,远中面为:0.0303±0.025,P>0.05;修复体邻面接触区黄色区域与绿色区域面积比值在就位顺利组中为:0.0234±0.013,就位困难组为:0.165±0.023,P<0.05;试戴时就位的难易程度与图像中黄色区域/绿色区域的面积比呈正相关(R=0.716,P<0.05)。结论高嵌体试戴就位顺利需要技师设计修复体邻面接触区时充分考虑到基牙、邻牙、对颌牙、冠修复外形的情况,同时还要做好医技沟通。 Objective To provide basis and guidance for the design of onlay interproximal interfaces and long-term clinical effects,we analyze the results of onlay try in made by CAD/CAM system.Methods A retrospective study was done in 92 patients who underwent onlay treatment from January 2017 to January 2019,which is divided into two groups.Group A was the onlay try it smoothly(46 high inlay)and group B was the onlay try in(46 high inlay)difficulty which needed to grinding interproximal surface porcelain.Comparing the area ratio of the yellow area to the green area,analysis the relevant factors influencing the effect of onlay try in.Results The area ratio of the yellow area to the green area in the adjacent contact area of the restoration was 0.0378±0.0261 in the mesial surface and 0.0303±0.025 in the distal surface,P>0.05.The area ratio of the yellow area to the green area in the adjacent contact area of the restoration body was 0.0234±0.013 in the smooth group and 0.165±0.023 in the difficult group,P<0.05.The difficulty of onlay try in is positively correlated with the area ratio of the yellow area to the green area(R=0.716,P<0.05).Conclusion It is necessary for technicians fully consider abutment teeth,adjacent teeth,matching teeth and crown prosthesis appearance,also make fully communication with the doctor,when design the adjacent contact area of the prosthesis..
