延续护理对脑卒中患者精神症状及并发症的影响

Effect of continuous nursing on mental symptoms and complications of stroke patients

目的为脑卒中患者提供降低和缓解精神症状及并发症的护理方法。方法选取2015年6月~2017年6月该院经过治疗后的脑卒中患者118例作为研究对象。根据采用的护理方法不同将符合纳入标准的118例患者分为研究组(65例)和对照组(53例)。研究组接受延续护理和对照组接受常规护理。通过随访统计两组患者在出院后6个月内精神症状类型、并发症、复发情况,并对患者护理前后对治疗的依从性及生存质量进行调查评分。结果研究组患者出院后6个月内出现思维障碍、意识障碍、感觉与直觉障碍、情绪异常、肺部感染、尿路感染、压力性损伤、消化道出血、再出血及复发的发生率均低于对照组,差异有统计学意义(均P<0.05)。研究组患者护理后治疗依从性、心理领域、生理领域、环境领域、社会关系领域的评分明显高于对照组,差异均有统计学意义(均P<0.05)。结论延续护理可以明显降低脑卒中患者精神症状和并发症的发生率,提高患者对治疗的依从性和生活质量。

Objective To provide nursing methods to reduce and relieve mental symptoms and complications for patients with cerebral apoplexy. Methods A total of 118 cases of stroke patients treated in this hospital from June 2015 to June 2017 were selected as the research objects. According to different nursing methods, 118 patients meeting the inclusion criteria were divided into study group (65 cases) and control group (53 cases). The study group received continuous care. The control group received routine care. The types of mental symptoms, complications and recurrence of the two groups of patients within 6 months after discharge were counted by follow-up, and the compliance with treatment and quality of life before and after nursing were investigated and scored. Results The incidence of thinking disorder, consciousness disorder, sensory and intuitive disorder, emotional abnormality, pulmonary infection, urinary tract infection, stress injury, gastrointestinal bleeding, rebleeding and recurrence in the study group were lower than those in the control group within 6 months after discharge, and the differences were statistically significant ( P<0. 05 ). The scores of patients' post-nursing compliance, psychological field, physiological field, environmental field and social relationship field in the study group were significantly higher than those in the control group, and the differences were statistically significant ( P<0. 05). Conclusions Continuous nursing can significantly reduce the incidence of mental symptoms and complications in stroke patients and improve the patientsx compliance with treatment and quality of life.
