腹腔镜胆总管探查术与开腹胆总管探查术89例分析

Analysis of 89 cases of laparoscopic common bile duct exploration and open bile duct exploration

目的探讨腹腔镜胆总管探查术(LCBDE)与开腹胆总管探查术(OCBDE)的临床应用区别。方法回顾性分析我院2016年4月至2018年10月期间胆总管手术共89例,LCBDE38例,OCBDE51例,分析比较两组患者在手术时间、术中出血量、胃肠功能恢复时间、术后住院时间、胆漏、结石残留并发症的差异。结果两组在手术时间、术后胆漏、术中出血量、结石残留的差异不具有统计学意义(P>0.05),两组在胃肠功能恢复、术后住院时间、术后疼痛、术口脂肪液化感染的差异有统计学意义(P<0.05)。结论LCBDE创伤小、恢复快、病人痛苦小,并发症少,临床有实际应用价值。

Objective To explore the clinical application of laparoscopic common bile duct exploration (LCBDE) and open bile duct exploration (OCBDE).Methods A retrospective analysis of our hospital from April 2016 to October 2018 a total of 89 cases of common bile duct surgery,LCBDE 38 cases,OCBDE 51 cases,analysis of two groups of patients in the operation time,blood loss, gastrointestinal function recovery time,surgery after hospital stay,bile leakage,residual complications of stones.Results There was no significant difference between the two groups in operation time,postoperative bile leakage,intraoperative blood loss and residual stones (P > 0.05).The recovery of gastrointestinal function,postoperative hospital stay,postoperative pain,lipid liquefaction infection was statistically significant difference (P < 0.05).Conclusions LCBDE is less traumatic,faster recovery,less pain and fewer complications,and has clinical value.
